Adverse Childhood Experiences (ACEs) refer to traumatic or stressful events that occur during childhood (usually before age 18), such as physical, emotional, or sexual abuse, neglect, and family dysfunction (domestic violence, addiction, parental mental illness, conflictual separations, etc.). Numerous studies, starting with the famous "ACE Study" conducted by the CDC and Kaiser Permanente in the 1990s, have shown that such experiences have profound and long-lasting effects on physical and mental health in adulthood. These findings indicate the need to identify these adverse childhood experiences early, particularly not only in psychological but also biological terms, given the recent literature on metabolic, epigenetic, and brain alterations. This focus requires an important interdisciplinary approach in treating these minors' issues, both psychologically, socially, and medically. The focus on cardiovascular diseases was chosen because they are the most common diseases in the international and Italian population and are the leading cause of mortality. Therefore, even clinically, the presence of unresolved issues related to these adverse experiences has been detected in the adult population affected by cardiovascular diseases, which also impact the ability to cope with treatments.
Le Esperienze Infantili Avverse (ACE – Adverse Childhood Experiences) si riferiscono a eventi traumatici o stressanti che avvengono durante l’infanzia (di solito prima dei 18 anni), come abusi fisici, emotivi o sessuali, trascuratezza, e disfunzioni familiari (violenza domestica, dipendenze, malattie mentali genitoriali, separazioni conflittuali, ecc.). Numerose ricerche, a partire dal celebre “ACE Study” condotto dai CDC e Kaiser Permanente negli anni ’90, hanno mostrato che tali esperienze hanno effetti profondi e duraturi sulla salute fisica e mentale in età adulta. I risultati indicano la necessità di individuare precocemente queste esperienze avverse infantili, in particolare non solo in termini psicologici ma anche biologici, data la recente letteratura di alterazioni metaboliche, epigenetiche e cerebrali. Tale attenzione implica un importante approccio interdisciplinare nel trattamento delle problematiche di questi minori sia a livello psicologico e sociale che medico. Il focus sulle malattie cardiovascolari è stato scelto perché sono le malattie più diffuse nella popolazione internazionale e italiana e sono alla base della più rilevante causa di mortalità. Pertanto, anche clinicamente, si è rilevato nella popolazione adulta affetta da malattie cardiovascolari la presenza di problematiche irrisolte legate a queste esperienze avverse che incidono anche sulla capacità ad affrontare i percorsi terapeutici.
