Exophthalmos is a clinically significant manifestation associated with various orbital and systemic diseases, and its quantitative assessment is essential for monitoring therapeutic effectiveness. Conventional measurement techniques, however, present limitations in terms of objectivity, reproducibility, repeatability and ease of use in routine clinical practice. This thesis presents the development of a prototype system for acquiring, registering, and automatically recognising the cornea in facial images, aimed at enabling non-invasive evaluation of ocular morphological changes associated with exophthalmos. The proposed system integrates a processing pipeline comprehending: standardised facial image acquisition, automated segmentation and identification of the corneal region, and spatial registration procedures to allow longitudinal comparison of images acquired at different time reference points. The prototype was evaluated on a cohort of sane subjects, assessing both the accuracy of corneal detection and the system’s sensitivity in identifying meaningful changes over time. Preliminary results demonstrate that the proposed approach enables objective and reproducible measurement of ocular modifications, supporting clinicians in evaluating therapeutic outcomes. The developed prototype represents a promising decision-support tool in ophthalmological practice and provides a foundation for future integration with advanced image processing and artificial intelligence–based analytical systems.
L'esoftalmo rappresenta una manifestazione clinica significativa associata a diverse patologie orbitali e sistemiche, e la sua valutazione quantitativa è essenziale per monitorare l'efficacia terapeutica. Tuttavia, le tecniche di misurazione convenzionali presentano limiti in termini di oggettività, riproducibilità, ripetibilità e facilità d'uso nella pratica clinica quotidiana. Questa tesi presenta lo sviluppo di un prototipo per l'acquisizione, la registrazione e il riconoscimento automatico della cornea in immagini facciali, volto a consentire una valutazione non invasiva delle alterazioni morfologiche oculari associate all'esoftalmo. Il sistema proposto integra una pipeline di elaborazione che comprende: l'acquisizione standardizzata di immagini facciali, la segmentazione e l'identificazione automatizzata della regione corneale e procedure di registrazione spaziale per permettere il confronto longitudinale di immagini acquisite in diversi momenti temporali. Il prototipo è stato valutato su una coorte di soggetti sani, analizzando sia l'accuratezza del rilevamento corneale sia la sensibilità del sistema nell'identificare cambiamenti significativi nel tempo. I risultati preliminari dimostrano che l'approccio proposto consente una misurazione oggettiva e riproducibile delle modificazioni oculari, supportando i clinici nella valutazione degli esiti terapeutici. Il prototipo sviluppato rappresenta un promettente strumento di supporto alle decisioni nella pratica oftalmologica e pone le basi per una futura integrazione con sistemi avanzati di elaborazione delle immagini e di analisi basata sull'intelligenza artificiale.
