Background. Orthorexia nervosa (ON) is a condition characterized by an excessive pursuit of eating habits perceived as perfectly pure and healthy. The impairments associated with ON suggest that it may function as a maladaptive coping strategy linked to negative internal experiences and adverse childhood experiences, in line with the assumptions of Schema Therapy. Indeed, this model has proven useful in understanding dysfunctional eating behaviors through the concept of Schema Modes, namely emotional and cognitive states resulting from the activation of early maladaptive schemas and the corresponding coping responses enacted in reaction to them. The aim of the present study was to profile ON from the perspective of Schema Modes. Methods. The study involved a sample of 374 participants (87.7% women) with a mean age of 42.72 years (SD = 12.89). Participants completed the Düsseldorf Orthorexia Scale (DOS), the Schema Mode Inventory–Eating Disorder–Short Form (SMI-ED-SF), and the Cognitive Restraint subscale of the Three-Factor Eating Questionnaire–Revised–18 (TFEQ-R-18). The sample was divided into six groups based on: (1) ON scores above the DOS cut-off; (2) equal cognitive restraint scores to those of the first group but ON scores below the cut-off; (3) equal sociodemographic characteristics to the first group but subthreshold ON scores; and three randomized control groups (4, 5, and 6). Differences between groups were examined using Profile Analysis. Results. Significant differences emerged across several Schema Modes. Specifically, the ON group showed significantly higher scores on the Demanding Parent, Punitive Parent, Vulnerable Child, and Eating Disorder Overcontroller modes, and lower scores on modes associated with impulsivity and poor self-control. Overall, ON was characterized by a profile marked by elevated self-criticism, emotional vulnerability, rigid standards, and a strong need for control. Conclusions. These findings suggest that ON is associated with the activation of specific Schema Modes, reflecting the use of maladaptive coping strategies that may contribute to the maintenance of orthorexic symptoms. In this regard, Schema Therapy techniques may represent a particularly promising approach for the treatment of ON.

Le compromissioni associate all’ON, la configurano come una possibile strategia di coping disfunzionale, associata a vissuti interni negativi ed esperienze infantili avverse, avvicinandosi a quanto proposto dalla Schema Therapy. Infatti, questo modello ha dimostrato la sua utilità nella comprensione dei comportamenti alimentari disfunzionali, tramite gli Schema Modes: stati emotivi e cognitivi risultanti dall’attivazione di schemi maladattivi precoci e dai corrispondenti coping agiti in risposta. L’obiettivo del presente studio era di profilare l’ON alla luce degli Schema Modes. Metodi. Lo studio ha coinvolto un campione di 374 soggetti, (87.7% donne) con età media di 42.72 anni (SD = 12.89). Sono stati somministrati la Düsseldorf Orthorexia Scale (DOS), lo Schema Mode Inventory-Eating Disorder- Short Form (SMI-ED-SF) e la scala Cognitive Restraint del Three Factor Eating Questionnaire – Revised – 18 (TFEQ-R-18). Il campione è stato diviso in sei gruppi, raggruppati per: (1) punteggi di ON superiori al cut-off della DOS, (2) punteggi uguali di cognitive restraint rispetto al primo gruppo ma punteggi di ON sotto il cut-off (3) uguali caratteristiche sociodemografiche al primo gruppo ma punteggi di ON sotto-soglia e tre gruppi di controllo randomici (4, 5, 6). Le differenze tra i gruppi sono state analizzate mediante Profile Analysis. Risultati. I gruppi hanno presentato differenze significative in diversi Schema Modes. In particolare, il gruppo con ON ha mostrato punteggi significativamente più elevati nel Demanding e Punitive Parent, Vulnerable Child ed Eating Disorder Overcontroller, e punteggi inferiori nei modes associati a impulsività e scarso autocontrollo. Nell’ON emerge un profilo caratterizzato da elevata autocritica, vulnerabilità emotiva, standard rigidi e bisogno di controllo. Conclusioni. Ciò suggerisce l’associazione tra ON e l’attivazione di specifici Schema Modes, riflettendo il ricorso a strategie di coping maladattive, che potrebbero mantenere l’ON. In relazione a questo, le tecniche della Schema Therapy si potrebbero configurare come particolarmente efficaci per il trattamento dell’ON.
