Pain is always a complex and multidimensional experience. Chronic pain from central sensitization, induced by neuroinflammation, becomes a pathology in itself: nociplastic pain which highlights the plastic, functional and structural changes that have occurred in the nervous system as a form of adaptation to all the different environmental signals. These changes involve not only different brain regions, but also various and entire neural networks, altering the overall dynamics of the connectome. This pathological process induces widespread dysfunctional changes, altering cognitive abilities, perception (not only of pain), emotional processing and the affective and relational sphere. All these are added to the chronic physical pain, significantly impacting overall quality of life. Epigenetic and molecular mechanisms that allow both normal and pathological neuroplastic alterations allow us to act with various integrated treatments, at different levels, in the attempt to restore optimal balance to the system. In this plastic recovery intervention, research on immature neurons is also considered: are they a resource for neuroplasticity? Do they act spontaneously physiologically? Do they contribute to recovery, enhancing the results of treatments?
Il dolore è sempre un’esperienza complessa e multidimensionale. Il dolore cronico da sensibilizzazione centrale, indotta da neuroinfiammazione, diventa una malattia in sé: il dolore nociplastico che evidenzia i cambiamenti plastici, funzionali e strutturali, avvenuti nel sistema nervoso come forma di adattamento a tutti i diversi segnali ambientali. In queste modifiche, non sono coinvolte solo diverse regioni cerebrali, bensì varie ed intere reti neurali, modificando le dinamiche complessive del connettoma. Questo processo patologico induce cambiamenti disfunzionali diffusamente, alterando le capacità cognitive, la percezione (non solo del dolore), l’elaborazione delle emozioni, la sfera affettiva e relazionale e si sovrappone al dolore fisico cronico, impattando in modo rilevante sulla qualità della vita in generale. Meccanismi epigenetici e molecolari che consentono le alterazioni neuroplastiche sia normali che patologiche, ci consentono di agire con vari trattamenti integrati a diversi livelli, nel tentativo di riportare un equilibrio ottimale nel sistema. In questo intervento di recupero plastico, si considerano anche le ricerche sui neuroni immaturi: sono una risorsa per la neuroplasticità? Agiscono in modo fisiologico spontaneo? Danno un contributo al recupero, implementando i risultati dei trattamenti?
