This thesis analyses the structural reform of local healthcare services within the strategic framework of the National Recovery and Resilience Plan, with a specific focus on the transition toward a "proximity-based" healthcare model. The study explores the implementation of Community Houses in the Veneto Region, analyzing the pivotal role of the Single Point of Access as the cornerstone of socio-health integration. The core of the research consists of an operational analysis conducted within a Local Health Authority, utilizing process mapping of internal care pathways. This study aims to evaluate the pre-reforms scenario against the evolving model, demonstrating how the reorganization of clinical and information workflows is essential to optimizing service access, enhancing coordination across the territorial network, and ensuring an effective response to the community's healthcare needs.
