Aim of the study The study analyzes the effects of a 10-day ketogenic diet on metabolism, body composition, and performance in endurance athletes, comparing male and female athletes. The ketogenic diet (KD) promotes ketosis and fat oxidation, but its effects on performance remain controversial. The aim is to evaluate possible sex-specific differences in metabolic adaptations and aerobic performance. Materials and methods Randomized controlled clinical trial involving 42 endurance cyclists, of whom 32 completed the study, assigned for 10 days to either a ketogenic or Mediterranean diet following a one-week run-in period. Body composition, metabolism, VO₂ max, time to exhaustion (TTE), metabolic parameters, and mood state were assessed. In male subjects, muscle biopsies and blood analyses were also performed. Results The ketogenic diet significantly increased ketonemia and fat oxidation while reducing the respiratory quotient, thereby promoting the use of fat as the primary energy substrate. Reductions in body weight and fat mass were observed, with more pronounced metabolic adaptations in women during exercise. The ketogenic diet elicited marked inter-individual variability in endurance performance. Despite a trend toward reduced performance compared with the high-carbohydrate diet, the pooled analysis of men and women did not demonstrate a statistically significant effect. Conclusion A short-term ketogenic diet induced favorable metabolic adaptations in endurance athletes, increasing lipid oxidation and improving body composition. However, these effects did not enhance aerobic performance, resulting in a reduction in time to exhaustion. Women also showed more pronounced metabolic adaptations than men, suggesting potential sex-specific differences in the response to the ketogenic diet.
Scopo dello studio La dieta chetogenica (KD), caratterizzata da una severa restrizione dei carboidrati (<5% dell’energia giornaliera), aumenta l’ossidazione dei lipidi e la produzione di corpi chetonici. Tuttavia, i suoi effetti sulla prestazione di endurance e le potenziali differenze sesso-specifiche nella risposta metabolica rimangono poco definiti. La presente tesi ha investigato gli effetti di una dieta chetogenica di 10 giorni sul metabolismo energetico, sulla composizione corporea e sulla performance aerobica in atleti di endurance, valutando eventuali differenze tra uomini e donne. L’obiettivo è valutare eventuali differenze sesso-specifiche negli adattamenti metabolici e nella performance aerobica. Materiali e metodi Studio clinico randomizzato e controllato su 42 ciclisti endurance, di cui 32 completanti, assegnati per 10 giorni a dieta chetogenica o mediterranea dopo una settimana di run-in. Prima e dopo l’intervento dietetico sono stati valutati i parametri di composizione corporea, metabolismo a riposo e durante l’esercizio e performance di endurance. Nei soggetti uomini sono state inoltre effettuate biopsie muscolari ed analisi ematiche. Risultati La dieta chetogenica ha aumentato significativamente la chetonemia e l'ossidazione lipidica, riducendo il quoziente respiratorio e promuovendo un maggiore utilizzo dei lipidi come substrato energetico. Gli adattamenti metabolici sono risultati più marcati nelle donne durante l'esercizio. Inoltre, la KD ha determinato una riduzione significativa del peso corporeo e della massa grassa. La risposta individuale è risultata altamente eterogenea e, sebbene la performance di endurance tendesse a ridursi rispetto alla dieta ad alto contenuto di carboidrati, tale differenza non ha raggiunto la significatività statistica nell'analisi complessiva di uomini e donne. Conclusione Una dieta chetogenica di breve durata ha indotto adattamenti metabolici favorevoli negli atleti di endurance, aumentando l’ossidazione lipidica e migliorando la composizione corporea. Tuttavia, tali adattamenti non si sono tradotti in un miglioramento della performance aerobica e, nonostante un’elevata eterogeneità della risposta individuale è stata osservata una tendenza a una riduzione della performance di endurance. Le donne hanno mostrato inoltre adattamenti metabolici più marcati rispetto agli uomini, suggerendo potenziali differenze sesso-specifiche nella risposta alla KD.
