Obesity is a chronic multifactorial disease associated with numerous cardiometabolic complications and a progressive deterioration in body composition. Although GLP-1 receptor agonists currently represent one of the most effective pharmacological strategies for weight loss, their use may also be associated with a reduction in muscle mass, an increased risk of sarcopenic obesity, and frequent long-term weight regain. The aim of this thesis is to analyze the effectiveness of a multidisciplinary approach based on GLP-1 pharmacological therapy, nutritional intervention, and structured physical exercise in improving body composition and maintaining long-term results compared with pharmacological therapy alone. This thesis is based on a narrative review of the scientific literature conducted mainly through the PubMed database, including recent systematic reviews, meta-analyses, and randomized controlled trials related to obesity, GLP-1 therapy, physical exercise, nutrition, and sarcopenic obesity. The analyzed evidence shows that the combination of pharmacological therapy, diet, and physical exercise is more effective than pharmacological treatment alone, as it promotes greater preservation of muscle mass, reduces the risk of sarcopenic obesity, and improves long-term weight loss maintenance and metabolic outcomes. In particular, resistance training and combined exercise appear to play a central role in counteracting the loss of lean mass associated with weight reduction. In conclusion, the multidisciplinary approach currently represents the most effective and sustainable strategy for obesity treatment, as it improves not only weight loss but also body composition quality and long-term maintenance of therapeutic outcomes.
L’obesità è una patologia cronica multifattoriale associata a numerose complicanze cardiometaboliche e a un progressivo peggioramento della composizione corporea. Sebbene gli agonisti del recettore del GLP-1 rappresentino oggi una delle strategie farmacologiche più efficaci per la perdita di peso, il loro utilizzo può associarsi anche a una riduzione della massa muscolare e a un aumentato rischio di obesità sarcopenica, oltre a un frequente recupero ponderale nel lungo termine. L’obiettivo della presente tesi è analizzare l’efficacia di un approccio multidisciplinare integrato, basato su terapia farmacologica con GLP-1, intervento nutrizionale ed esercizio fisico, nel miglioramento della composizione corporea e nel mantenimento dei risultati nel tempo rispetto alla sola terapia farmacologica. La tesi si basa su una revisione narrativa della letteratura scientifica condotta principalmente attraverso il database PubMed, includendo revisioni sistematiche, meta-analisi e randomized controlled trials recenti relativi a obesità, GLP-1, esercizio fisico, nutrizione e obesità sarcopenica. Le evidenze analizzate mostrano che l’associazione tra terapia farmacologica, dieta ed esercizio fisico risulta più efficace rispetto al solo trattamento farmacologico, poiché favorisce una maggiore preservazione della massa muscolare, una riduzione del rischio di obesità sarcopenica e un miglior mantenimento della perdita di peso e dei benefici metabolici nel lungo termine. In particolare, l’esercizio di forza e l’allenamento combinato sembrano svolgere un ruolo centrale nel contrastare la perdita di massa magra associata al dimagrimento. In conclusione, l’approccio multidisciplinare rappresenta attualmente la strategia più efficace e sostenibile per il trattamento dell’obesità, in quanto consente di migliorare non solo la perdita di peso, ma anche la qualità della composizione corporea e la stabilità dei risultati terapeutici nel tempo.
