Atrial fibrillation is one of the most common cardiac arrhythmias and is frequently treated by catheter ablation through electrical isolation of the pulmonary veins. Among the most recent techniques, Pulsed Field Ablation (PFA) is based on irreversible electroporation of myocardial tissue and requires an adequate electrode–tissue interaction to ensure effective energy delivery. In this context, the aim of this thesis is to develop a methodology for the characterization of the electrode portion that is effectively active during the procedure, with particular reference to multielectrode catheters used for the treatment of atrial fibrillation. The study includes a review of the physical and instrumental principles underlying complex impedance measurement and the assessment of electrode–tissue proximity/contact, followed by the analysis of technical data exported from clinical procedures. The objective is to investigate the relationship between the available electrical parameters, the degree of electrode contact, and the possible estimation of the active electrode portion, in order to contribute to a better interpretation of catheter–tissue interaction and, ultimately, of the local distribution of therapeutic intensity.
La fibrillazione atriale rappresenta una delle aritmie cardiache più diffuse ed è frequentemente trattata mediante ablazione transcatetere con isolamento elettrico delle vene polmonari. Tra le tecniche più recenti, la Pulsed Field Ablation (PFA) si basa sull’elettroporazione irreversibile del tessuto miocardico e richiede un’adeguata interazione tra elettrodo e tessuto per garantire un’erogazione efficace della terapia. In questo contesto, il presente lavoro si propone di sviluppare una metodologia per la caratterizzazione della porzione di elettrodo effettivamente attiva durante la procedura, con particolare riferimento a cateteri multielettrodo impiegati nel trattamento della fibrillazione atriale. Lo studio prevede una revisione dei principi fisici e strumentali alla base della misura impedenziometrica complessa e della valutazione di prossimità/contatto elettrodo-tessuto, seguita dall’analisi di dati tecnici esportati da procedure cliniche. L’obiettivo è investigare la relazione tra i parametri elettrici disponibili, il grado di contatto degli elettrodi e la possibile stima della porzione attiva di elettrodo, al fine di contribuire a una migliore interpretazione dell’interazione catetere-tessuto e, in prospettiva, della distribuzione locale dell’intensità terapeutica.
