Haemophilia is a rare genetic disease that can cause serious musculoskeletal complications and it can drastically reduce the quality of life of those who are affected. Recent scientific evidence suggests that physical exercise can have a positive impact on the treatment of people with haemophilia. However, there are still no general guidelines regarding the frequency, intensity, and type of exercise for haemophilia patients. The aim of this study is to perform a literature review aimed to investigate the effects that exercise can have on patients with haemophilia. In particular, the research aims to analyse the different types of physical exercise, including aerobic, strength, combined (aerobic and strength), balance and aquatic exercise, and the evaluation of any benefits and risks that these activities produce in haemophilic subjects. The bibliographic research was carried out using the main online databases, namely PubMed, Scopus, and Web of Science; the keywords used for the research were "haemophilia" and “hemophilia” related to "exercise", "physical activity" and "sports". As an initial result, 645 articles were found; however, only ten studies were included in the final review as they met the research inclusion and exclusion criteria. Through these studies it was possible to investigate the role of physical exercise on the treatment of subjects with haemophilia. Most of the studies used assessment tools such as questionnaires and scales to evaluate quality of life and general health, including Haemophilia-specific health-related quality of life (Haem-A-QoL) and the Haemophilia Joint Health Score (HJHS), or tests for the evaluation of functional capacity such as the Six Minute Walking Test (6MWT), or again, tests for the evaluation of muscle strength and joint Range of Motion (ROM) through hand grips and specific machinery. In conclusion, it can be said that most of the studies have shown that physical exercise is safe and it has positive effects in subjects suffering from haemophilia; in addition to this, it would be necessary to emphasize the importance of both the promotion and the prescription of physical exercise in the treatment of patients with haemophilia.

L’emofilia è una malattia genetica rara che può causare gravi complicazioni a livello muscolo-scheletrico e ridurre drasticamente la qualità di vita delle persone che ne sono affette. Le recenti evidenze scientifiche ci suggeriscono che l’esercizio fisico può avere un effetto positivo nel trattamento dei soggetti affetti da emofilia. Tuttavia, non vi sono ancora linee guida generali circa la frequenza, l’intensità e la tipologia di esercizio fisico per i pazienti emofilici. Lo scopo di questo studio è quello di eseguire una revisione della letteratura volta ad analizzare gli effetti che l’esercizio fisico può apportare nei pazienti affetti da emofilia. In particolare, la ricerca ha come obiettivo primario l’analisi delle diverse tipologie di esercizio fisico, quali l’esercizio di tipo aerobico, di forza, combinato (aerobico e forza), di equilibrio ed esercizio in ambiente acquatico; in secondo luogo, lo studio valuta gli eventuali benefici e i rischi che queste attività fisiche comportano nei soggetti emofilici. La ricerca bibliografica è stata svolta mediante l’utilizzo dei principali database presenti online, ovvero PubMed, Scopus e Web of Science; le parole chiave utilizzate per la ricerca sono state “haemophilia” e “hemophilia” correlata a “exercise”, “physical activity” e “sports”. Dei 645 articoli inizialmente riscontrati, solo dieci studi sono stati inclusi nella revisione finale in quanto conformi ai criteri di inclusione ed esclusione della ricerca. L’analisi di questi lavori ha permesso di approfondire l’importanza dell’esercizio fisico per i soggetti emofilici e il ruolo complementare che può rivestire nell’ambito del trattamento di questi pazienti. La maggior parte degli studi utilizzava come strumenti di valutazione questionari, scale della qualità di vita e della salute generale, tra cui l’Haemophilia-specific health-related quality of life (Haem-A-QoL) o l’Haemophilia Joint Health Score (HJHS), oppure test per la valutazione della capacità funzionale come il Six Minute Walking Test (6MWT) o per quantificare la forza muscolare e il Range of Motion (ROM) articolare attraverso hand grip e macchinari specifici. In conclusione, si può affermare che gran parte degli studi hanno dimostrato che l’esercizio fisico è sicuro e comporta effetti positivi nei soggetti affetti da emofilia. La promozione e la prescrizione dell’esercizio fisico dovrebbe essere un punto cardine del trattamento di questi pazienti, in associazione con la terapia farmacologica.
