Background: the shortage of organ and tissue donors is undermining self-sufficiency even in developed countries. Epidemiological and demographic changes in the population have altered the profile of potential beating-heart donors. The Veneto Region has developed and approved a Diagnostic-Therapeutic Pathway (DTP) for the proper management and care of patients with severe acute brain injury and poor prognosis, in compliance with existing end-of-life care legislation and with the aim of increasing the current number of potential donors. Aims: the purpose of this study is to investigate the knowledge of healthcare professionals (neurologists/neurophysiologists and neurophysiology technicians) regarding the determination of death by neurological criteria, as well as organ and tissue donation and retrieval, and to assess their knowledge and perception of the regional DTP in a territorial hospital setting. The survey could highlight potential obstacles to the implementation of the pathway. Methods: in June 2023, an observational, prospective, analytical study was conducted by administering a questionnaire in the Complex Operating Unit of Neurology at the Legnago Hospital, ULSS 9 Healthcare Company. Results: the data from this study, although obtained from a small sample size, show a favorable attitude toward organ donation, both in general terms and on a personal level. The analyzed data indicate the level of training possessed by the healthcare personnel in terms of determining death by neurological criteria and their familiarity with the regional DTP. These elements are considered a valid strategy for increasing the number of potential beating-heart organ and tissue donors. Conclusions: the analysis and understanding of the factors that impact the implementation of such a pathway, as highlighted by the professionals involved, can help improve the pathway, rationalize resources, and optimize outcomes.
Background: la carenza di donatori di organi e tessuti sta minando l’autosufficienza anche dei Paesi sviluppati e i cambiamenti epidemiologici e demografici della popolazione hanno modificato il profilo del potenziale donatore a cuore battente. La Regione Veneto ha elaborato ed approvato un Percorso Diagnostico Terapeutico (PDT), al fine della corretta gestione e presa in carico del paziente con grave cerebrolesione acuta a prognosi infausta, nel rispetto della normativa vigente in tema di cure di fine vita e per incrementare l’attuale numero di potenziali donatori. Obiettivi: scopo dell’elaborato è indagare la conoscenza dei Professionisti sanitari (Neurologi/Neurofisiopatologi e Tecnici di Neurofisiopatologia) sulla determinazione di morte con criteri neurologici, la donazione e il prelievo di organi e tessuti, nonché la conoscenza e la percezione in merito al PDT regionale. I dati ottenuti potrebbero consentire di evidenziare eventuali criticità, motivo di ostacolo alla realizzazione del percorso stesso. Materiali e Metodi: a giugno 2023, è stato, pertanto, condotto uno studio osservazionale, prospettico, analitico e trasversale, mediante la somministrazione di un questionario, nell’Unità Operativa Complessa di Neurologia del presidio ospedaliero di Legnago, Azienda ULSS 9. Risultati: i dati di questo studio, seppur rilevati su un campione numerico ridotto, mostrano una favorevole tendenza alla donazione degli organi, sia sul piano generale, che personale. Dai dati analizzati emerge la formazione di cui tale personale dispone, sia per quanto concerne la determinazione di morte con criteri neurologici sia per il PDT regionale. La formazione è ritenuta una valida strategia per aumentare i potenziali donatori di organi e tessuti a cuore battente. Conclusioni: l’analisi e la conoscenza dei fattori che impattano sulla realizzazione di un tale percorso, messi in evidenza dagli stessi Professionisti coinvolti, possono consentire di migliorare il processo, razionalizzando le risorse ed ottimizzando i risultati.
