Aim: This study aims to investigate the possible association between nursing staffing characteristics, and healthcare-associated infections (HAIs) in Italian Pediatric Intensive Care Units (PICUs) that adhere to the Tipnet registry, a national web-based register. Materials and Methods: Data on PICU admissions, as well as patients’ clinical characteristics (including outcomes), and some select features regarding the centres involved in the study were extracted from the Tipnet registry and used for analysis. A survey was administered to healthcare personnel who worked in PICUs between the 1st of January 2023 and the 30th of June 2023. The objective was to collect data regarding the healthcare professionals and some characteristics of the centres (information not included in the Tipnet register). Generalized Estimating Equation Models (GEE) were used to investigate the association between staffing characteristics along with the nurses’ level education, and care-related infections in PICUs. Results: After adjusting for age, gender and presence of chronic disease, the data analysis revealed a statistically significant direct correlation between nurse turnover rate and the likelihood of a patient developing HAI. The results also indicated that a higher level of postgraduate education was associated with a 2% lower average probability of developing HAI. However, this association was not statistically significant. Furthermore, the professional experience of nurses in PICU did not show a significant association with the risk of developing HAI. Conclusions: This study contributes to existing knowledge on the effect of staffing characteristics and training on patient adverse events, especially in the pediatric intensive care settings and in Italy.
Background: l'Unità di Terapia Intensiva Pediatrica (Tiped) è una struttura specializzata nella cura e l’assistenza a neonati, bambini e adolescenti in condizioni critiche, sia acute che croniche, in cui i professionisti sanitari si concentrano su monitoraggio, diagnosi e trattamento dei pazienti, al fine di perseguire i migliori risultati di cura, fornendo supporto avanzato alle vie aeree, al sistema emodinamico e agli organi vitali. Nonostante la riconosciuta importanza degli infermieri nell’erogazione dell’assistenza sanitaria, le prove riguardanti il loro impatto sugli esiti dei pazienti ricoverati in Terapia Intensiva Pediatrica rimangono limitate, sottolineando la necessità di comprendere l’importanza dell’assistenza infermieristica, nonché di dimostrare le possibili relazioni tra le caratteristiche dell’assistenza stessa e gli esiti dei pazienti, in un’ottica di miglioramento della qualità assistenziale. Obiettivo: il presente studio si pone l’obiettivo di indagare le possibili relazioni esistenti tra caratteristiche dello staffing, formazione degli infermieri ed esiti sensibili all’assistenza infermieristica nei reparti di Terapia Intensiva Pediatrica afferenti al registro nazionale web-based, prospettico e multicentrico denominato Tipnet. Gli esiti sensibili all’assistenza infermieristica principalmente indagati ed estraibili dal database Tipnet saranno i seguenti: - infezioni del flusso sanguigno associate al catetere venoso centrale (Catheter-Associated Bloodstream Infection - CLABSI) - infezioni del tratto urinario associate al catetere vescicale (Catheter-Associated Urinary Tract Infections – CAUTI) - polmoniti associate alla ventilazione meccanica (Ventilator-Associated Pneumonia). Materiali e metodi: i dati relativi alle caratteristiche dei pazienti, ad alcune caratteristiche dei centri e agli esiti saranno ricavati dal registro Tipnet (applicativo RedCap), considerando i dati relativi ad un periodo di tempo circoscritto. I dati relativi ad altre caratteristiche dei centri, alla formazione e all’esperienza degli infermieri che hanno prestato servizio presso i centri considerati nel periodo di riferimento saranno ricavati mediante la somministrazione di una survey online ai colleghi infermieri, ai coordinatori e ad un referente medico dei centri stessi.
