OBJECTIVE: To produce a synthesis of systematic reviews and meta-analyses concerning informal caregivers of cancer patients that have considered aspects of social support (social network, family, romantic, friendship or community ties, psychosocial interventions, etc.). METHOD: Databases were consulted: Pubmed, EMBASE, PsycINFO, CINAHL, Web of Science and Scopus. The search yielded 1377 titles. Eligibility criteria were: English language; informal caregivers and patients belonging to the adult population; being systematic reviews or meta-analyses with publication date from the year 2013. The JBI Critical Appraisal Checklist for Systematic Reviews and Reasearch Syntheses tool was used to assess the quality of systematic reviews. There were 32 articles that met the inclusion criteria and were analyzed. RESULTS: The results were organized into three main thematic areas. A first area contains the elements answering the question "what role does social support play in the experience of informal carers of cancer patients?". The ways in which the caregiver manages the support received and his/her strategies for seeking social support are also part of this area. The second thematic area describes the change in interpersonal relationships and the relationship with the patient. The third thematic area collects the unmet support needs of caregivers and the effectiveness of psychosocial interventions (psychoeducation; dyad; group; supportive nature or psychotherapy). CONCLUSIONS: Social support plays an important role in adapting to the caregiver role. The caregiver faces changes in interpersonal relationships and daily life. It is important to nurture the relationship with the patient both to improve the caregivers' experience and to protect them from possible difficulties in grieving. Psycho-educational interventions combined with supportive interventions have shown good effectiveness. The results of the study suggest an integrated approach that considers the patient-caregiver dyad and psychosocial interventions tailored to the specific needs of the dyad and the specific time in the cancer trajectory.
OBIETTIVO: Realizzare una sintesi delle revisioni sistematiche e metanalisi riguardanti i caregivers informali di pazienti oncologici che hanno considerato aspetti del supporto sociale (network sociale, legami familiari, romantici, amicali o di comunità, interventi psicosociali, etc.). METODO: Sono state consultate le banche dati: Pubmed, EMBASE, PsycINFO, CINAHL, Web of Science e Scopus. La ricerca ha prodotto 1377 titoli. I criteri di eleggibilità sono: lingua inglese; caregivers informali e pazienti appartenenti alla popolazione adulta; essere revisioni sistematiche e/o metanalisi con data di pubblicazione dall’anno 2013. È stato utilizzato lo strumento JBI Critical Appraisal Checklist for Systematic Reviews and Reasearch Syntheses per valutare la qualità delle revisioni sistematiche. Gli articoli che hanno soddisfatto i criteri di inclusione e sono stati analizzati sono 32. RISULTATI: I risultati sono stati organizzati su tre principali aree tematiche. Una prima area racchiude gli elementi che rispondono alla domanda “quale ruolo ha il supporto sociale nell’esperienza dei caregivers informali di pazienti oncologici?”. Fanno parte di questa area anche le modalità con cui il caregiver gestisce il supporto ricevuto e le sue strategie di ricerca di supporto sociale. La seconda area tematica descrive il cambiamento delle relazioni interpersonali e della relazione con il paziente. La terza area tematica raccoglie i bisogni di supporto non soddisfatti dei caregivers e l’efficacia degli interventi psicosociali (psico-educazione; dedicati alla diade; di gruppo; di natura supportiva o di psicoterapia). CONLUSIONI: Il supporto sociale ha un ruolo importante nell’adattamento al ruolo di caregiver. Il caregiver affronta cambiamenti nelle relazioni interpersonali e nella vita quotidiana. È importante curare la relazione con il paziente sia per migliorare l’esperienza dei caregivers che per proteggerli da eventuali difficoltà nell’elaborazione del lutto. Gli interventi di psico-educazione uniti a quelli di natura supportiva hanno mostrato una buona efficacia. I risultati dello studio suggeriscono un approccio integrato, che consideri la diade paziente-caregiver ed interventi psicosociali personalizzati sulla base delle esigenze specifiche della diade e del momento specifico della traiettoria del cancro.
