This work analyzes the effectiveness of Mentalization-Based Treatment, proposed by Anthony Bateman and Peter Fonagy, on patients diagnosed with Psychotic Spectrum Disorders, also referring to Martin Debbané's literature. To support this thesis, a clinical study was conducted on 9 patients with this diagnosis, who underwent both group and individual therapy for one year, aiming to enhance mentalizing skills. Patients underwent the same tests at the beginning and at the end of treatment to investigate any instances of physical abuse and assess reflective ability, relationship quality, and coping strategies implementation. Simultaneously, clinicians involved in this research administered a battery of tests to evaluate the evolution of symptoms and overall, social, and occupational functioning of each patient. Once all data were collected, descriptive statistics of the raw scores obtained at the beginning and end of treatment were calculated, and non-parametric comparisons were conducted between the results obtained at the two moments in time using Wilcoxon statistics. The study indicates that patients experienced improvement in expressing emotions, with a decrease in emotion identification. Clinicians observed a decrease in symptomatology of psychotic spectrum disorders, with normalization of positive and negative symptoms, and improvement in conceptual disorganization, flattened affect, apathy, and psychotic behaviors.
Questo elaborato analizza l’efficacia del trattamento basato sulla mentalizzazione, proposto da Anthony Bateman e Peter Fonagy, su pazienti con diagnosi di Disturbi dello Spettro Psicotico, facendo riferimento anche alla letteratura di Martin Debbané. Per avvalorare questa tesi, è stato condotto uno studio clinico su 9 pazienti con tale diagnosi, i quali hanno seguito sia una terapia di gruppo che individuale della durata di un anno, con il fine di intensificare la capacità mentalizzante. Ai pazienti sono stati somministrati gli stessi test all’inizio e alla fine del trattamento, atti ad indagare eventuali casi di abusi fisici e a valutare la capacità riflessiva, la qualità delle relazioni e la messa in atto delle strategie di coping. Contemporaneamente, i clinici che hanno aderito a questa ricerca hanno compilato una batteria di test per valutare l’evoluzione dei sintomi e del funzionamento generale, sociale e lavorativo di ciascun paziente. Una volta raccolti tutti i dati, sono state calcolate le statistiche descrittive dei punteggi grezzi ottenuti, sia all’inizio che alle fine del trattamento, e sono stati svolti confronti non parametrici tra i risultati emersi nei due tempi, tramite Wilcoxon. Dallo studio si evince che i pazienti abbiano riscontrato un miglioramento nell’espressione delle emozioni, con un calo nell’identificazione delle stesse. I clinici hanno riscontrato un calo della sintomatologia dei disturbi dello spettro psicotico, riscontrando una normalizzazione dei sintomi positivi e negativi, con un miglioramento della disorganizzazione concettuale, dell’appiattimento affettivo, dell’apatia e dei comportamenti psicotici.
