This thesis explores the topic of supporting individuals with intellectual disabilities throughout their life journey, with a specific focus on the synergy between family, social-health services, and the community. The primary objective is to understand how these actors can collaborate effectively to ensure adequate support, promoting autonomy and quality of life for individuals with disabilities. The first chapter provides an analysis of the roles and responsibilities of the key stakeholders: the family, as the primary source of support; social-health services, which offer specialized assistance and guidance; and the community, understood as a space for inclusion and active participation. The importance of a collaborative network capable of addressing various needs throughout different life stages is emphasized. The second chapter focuses on the process of transitioning from childhood to adulthood, a crucial phase that presents significant challenges for individuals with disabilities and those who support them. Fundamental topics such as building an independent life and the right to self-determination are examined. In this context, the concepts of empowering co-design and personalized life projects are analyzed, highlighting the importance of placing individuals with disabilities at the center of the decision-making process, respecting their wishes, needs, and potential. The third chapter presents a research study conducted within the Azienda ULSS 6 Euganea area, aimed at investigating the level of awareness and access to available services by families of individuals with intellectual disabilities. The study evaluates how effectively these services meet families' needs for information, guidance, and support. The findings highlight strengths, weaknesses, and gaps in the professional practices currently adopted, proposing organizational and operational solutions that better align with the needs expressed by families. This thesis aims to make a meaningful contribution to the discussion on improving policies and practices for supporting individuals with disabilities, promoting an integrated and participatory model that values the role of individuals with disabilities as active protagonists in their life journey.
La presente tesi esplora il tema dell’accompagnamento delle persone con disabilità intellettiva lungo il loro percorso di vita, ponendo particolare attenzione alla sinergia tra famiglia, servizi socio-sanitari e comunità. L’obiettivo principale è comprendere come questi attori possano collaborare per garantire un supporto efficace, promuovendo l’autonomia e la qualità della vita della persona con disabilità. Il primo capitolo analizza in dettaglio i ruoli e le responsabilità dei soggetti coinvolti: la famiglia, quale nucleo primario di sostegno, i servizi socio-sanitari, che forniscono assistenza specialistica e orientamento, e la comunità, intesa come spazio di inclusione e partecipazione attiva. Viene evidenziata l’importanza di una rete collaborativa capace di rispondere alle diverse esigenze durante le fasi della vita. Il secondo capitolo si concentra sul delicato processo di transizione dall’età evolutiva all’età adulta, un passaggio che rappresenta una sfida cruciale sia per la persona con disabilità sia per chi la supporta. Vengono approfonditi temi fondamentali quali la costruzione di una vita indipendente, il "Dopo di noi" e il diritto alla scelta autodeterminata. In questo contesto, si analizzano i concetti di co-progettazione capacitante e progetto di vita personalizzato, evidenziando l’importanza di mettere la persona con disabilità al centro del processo decisionale, nel rispetto dei suoi desideri, bisogni e potenzialità. Nel terzo capitolo, viene presentata una ricerca condotta sul territorio dell’Azienda ULSS 6 Euganea, volta a indagare il livello di conoscenza e accesso ai servizi da parte delle famiglie di persone con disabilità intellettiva. Lo studio mira a valutare come i servizi disponibili riescano a rispondere alle esigenze delle famiglie sul piano informativo, di orientamento e di sostegno. I risultati evidenziano punti di forza, criticità e lacune delle pratiche professionali attualmente adottate, suggerendo proposte organizzative e operative più aderenti alle necessità espresse dalle famiglie. Questa tesi intende offrire un contributo significativo al dibattito sul miglioramento delle politiche e delle pratiche per l’accompagnamento delle persone con disabilità, promuovendo un modello integrato e partecipativo che valorizzi il ruolo della persona con disabilità come protagonista del proprio percorso di vita.
