The efficacy of Intensive Short-Term Dynamic Psychotherapy (ISTDP) has been widely studied for a broad range of common mental disorders (Abbass, Kisely, Rasic & Katzman, 2013; Lilliengren, Johansson, Lindqvist, Mechler & Andersson, 2016). Its short duration forces the identification of a specific focus on which both therapist and patient work together. This raises doubts on the possibility for ISTDP to let countertransference dynamics emerge to be explored, as Jones claimed before (2000). Racker (1968) claimed the importance of countertransference as a crucial element within the whole psychoanalitic treatment, suggesting the impossibility to separate countertransference from technique. Consequently, the present study aims to answer to the following questions: do the countertransference aspects play a role within a short-term psychoanalitic treatment? If so, what role is it? If not, does this influence the outcome? With this purpose, we identified the interaction structures within an ISTDP treatment using the Psychotherapy Process Q-Sort (PQS). In fact, this construct conceptualizes specific psychoanalitic process aspects, such as countertransference dynamics. The present work falls in process-outcome research in psychotherapy and uses a single-case design.
L’efficacia delle Psicoterapie Dinamiche Brevi Intensive (ISTDP) è stata ampiamente studiata in letteratura per il trattamento di un gran numero di disturbi mentali comuni (Abbass, Kisely, Rasic & Katzman, 2013; Lilliengren, Johansson, Lindqvist, Mechler & Andersson, 2016). Il vincolo temporale previsto da tale approccio impone che questo si fondi necessariamente sull’individuazione di un focus specifico su cui terapeuta e paziente lavorano insieme. Ciò ha suscitato dubbi sulla possibilità dell’ISTDP di permettere l’emergere di dinamiche transferali e controtransferali e di accedervi, a partire da Jones (2000). L’importanza del controtransfert quale elemento decisivo per la conduzione della psicoterapia psicoanalitica nella sua interezza viene testimoniata dal lavoro di Racker (1968), suggerendo l’impossibilità di separare completamente il controtransfert dalla tecnica. Il presente studio si propone quindi di rispondere alle seguenti domande: all’interno di un trattamento analitico breve, gli aspetti relazionali riconducibili al controtransfert hanno un ruolo e, se sì, quale? In caso contrario, tale mancanza inficia l’esito del trattamento? A questo scopo, sono state identificate le strutture di interazione all’interno di un trattamento ISTDP attraverso lo Psychotherapy Process Q-Sort (PQS). Tale costrutto, infatti, rappresenta il tentativo di operazionalizzare aspetti del processo analitico quali appunto le dinamiche controtransferali. Il presente lavoro si inscrive nel filone di ricerca process-outcome in psicoterapia e adotta un metodo single-case.
