This thesis analyzes the experience of the Territorial Health Nodes in the Municipality of Rimini, local spaces that represent an innovative model of integration between health, social services, and the community. The Nodes serve as neighborhood reference points, easily accessible to residents, with the aim of promoting equity in access to care and health as a collective good. Within these spaces, professionals from various fields—family nurses, community psychologists, public health workers, social workers, micro-area workers, and neighborhood healthcare workers—work together in multidisciplinary teams to respond to people's needs in a coordinated, personalized, and integrated manner. The operating model promotes mutual understanding among professionals, networking, and active community participation. Each Node is equipped with dedicated spaces for listening, individual assistance, teamwork, and the development of group interventions geared toward prevention, health promotion, and co-planning with the community. The facilities include at least three operational rooms and a 70-square-meter meeting room, designed to host community activities and opportunities for discussion between residents and staff. Through the analysis of this model, this thesis aims to highlight the potential of Territorial Nodes as an effective tool for social and healthcare integration and for strengthening social cohesion at the local level.
La presente tesi analizza l’esperienza dei Nodi Territoriali di Salute nel Comune di Rimini, spazi di prossimità che rappresentano un innovativo modello di integrazione tra servizi sanitari, servizi sociali e comunità. I Nodi si configurano come punti di riferimento nei quartieri, facilmente accessibili ai cittadini, con l'obiettivo di favorire l’equità nell’accesso alle cure e promuovere la salute come bene collettivo. All’interno di questi spazi, professionisti di diversi ambiti – infermieri di famiglia, psicologi di comunità, operatori della salute pubblica, assistenti sociali, operatori di microzona e OSS di quartiere – collaborano in equipe multidisciplinari per rispondere in modo coordinato, personalizzato e integrato ai bisogni delle persone. Il modello operativo valorizza la conoscenza reciproca tra professionisti, il lavoro in rete e la partecipazione attiva della comunità. Ogni Nodo è dotato di ambienti dedicati all’ascolto, all’assistenza individuale, al lavoro d’équipe e allo sviluppo di interventi di gruppo orientati alla prevenzione, promozione della salute e co-progettazione con il territorio. Le strutture prevedono almeno tre stanze operative e una sala incontri di 70 mq, pensata per ospitare attività comunitarie e momenti di confronto tra cittadini e operatori. Attraverso l’analisi di questo modello, la tesi intende evidenziare le potenzialità dei Nodi Territoriali come strumento efficace di integrazione socio-sanitaria e di rafforzamento della coesione sociale a livello locale.
