With the emergence of the biomedical paradigm and the rise of Evidence-Based Medicine (EBM), medicine has gradually lost its character as an “art,” increasingly taking on the traits of a scientific discipline. This shift may encourage the adoption of reductive approaches; for example, there is a risk of viewing illness solely from a medical-biological perspective (disease), while neglecting the patient’s personal experience of illness (illness) and the social dimension of disease (sickness). To reconcile the scientific approach with a focus on the individual, the paradigm of Narrative-Based Medicine (NBM) has emerged, aiming to place the patient back at the centre of the care relationship. This thesis proposes a possible integration between EBM and NBM, aimed at developing care pathways that acknowledge and embrace the complexity of clinical practice. In this regard, the contribution of palliative care will be highlighted as an example of global care, focused on the well-being of the patient as a whole.
Con l’avvento del paradigma biomedico e l’affermarsi della Evidence-Based Medicine (EBM), la medicina ha progressivamente perso il carattere di “arte”, assumendo sempre di più i tratti di una disciplina scientifica. Questo cambiamento può alimentare l’adozione di approcci riduttivi; ad esempio, vi è il rischio di considerare la patologia dal punto di vista esclusivamente medico-biologico (disease), trascurando il vissuto di malattia del paziente (illness) e la dimensione sociale della malattia (sickness). Per riconciliare l’approccio scientifico con un’attenzione alla persona, si è sviluppato il paradigma della Narrative-Based Medicine (NBM), che mira a rimettere il paziente al centro della relazione di cura. In questa tesi si propone una possibile integrazione tra EBM e NBM, volta a sviluppare percorsi di cura che riconoscano e accolgano la complessità della pratica clinica. A tal proposito, verrà valorizzato il contributo delle cure palliative come esempio di cura globale, orientata al benessere del paziente nella sua interezza.
