Cannabis sativa L. is an annual herbaceous plant whose use has evolved over the years from traditional medicinal and ritual purposes to its current therapeutic and recreational use, sparking debate in scientific, social, and regulatory circles. This thesis analyzes the botanical, chemical, and pharmacological aspects of cannabis, focusing on its main constituents, namely tetrahydrocannabinol (THC) and cannabidiol (CBD), also including other varieties, namely Cannabis indica Lam. and Cannabis ruderalis Janish. Toxicokinetic and toxicodynamic mechanisms and the involvement of the endocannabinoid system will be discussed to highlight the therapeutic potential as well as the risks associated with consumption. The analysis includes potential clinical uses and the risks of abuse, addiction, and adverse reactions, with reference to the psychological and behavioral implications of consumption. It will also include an analysis of both national and international regulations, highlighting their differences and their social and cultural impacts, and will describe the position and role of cannabis in modern society. In conclusion, it will outline future prospects and possible developments in the scientific, clinical, and legislative fields, emphasizing how the fate of cannabis will depend on the balance between the recognition of its therapeutic potential and the informed management of the risks associated with its use.
Cannabis sativa L. è una pianta erbacea annuale il cui impiego si è evoluto negli anni dall’uso tradizionale a scopo medico e rituale fino all’attuale utilizzo terapeutico e consumo ricreativo, creando dibattito negli ambiti scientifico, sociale e normativo. Questa tesi vuole analizzare gli aspetti botanici, chimici e farmacologici della cannabis, focalizzandosi sui principali costituenti, ovvero tetraidrocannabinolo (THC) e cannabidiolo (CBD), facendo anche accenno alle altre varietà di essa ovvero Cannabis indica Lam. e Cannabis ruderalis Janish. Saranno trattati i meccanismi tossicocinetici e tossicodinamici e il coinvolgimento del sistema endocannabinoide per evidenziare le potenzialità terapeutiche ma anche i rischi legati al consumo. L’analisi comprende i potenziali impieghi clinici e i rischi dovuti ad abuso, dipendenza e reazioni avverse, con accenni anche alle implicazioni psicologiche e comportamentali dovuti al consumo. Sarà compresa inoltre un’analisi delle normative sia nazionali che internazionali, evidenziando differenze ed effetti di questi a livello sociale e culturale e verrà descritta la posizione e il ruolo della cannabis nella società moderna. In conclusione, verranno delineate delle prospettive future e possibili sviluppi in ambito scientifico, clinico e legislativo, sottolineando come il destino della cannabis dipenderà dall’equilibro tra il riconoscimento delle sue potenzialità terapeutiche e la gestione consapevole dei rischi associati al suo utilizzo.
