Background Triple-negative breast cancer (TNBC) represents a significant clinical challenge due to its clinic-biological characteristics, with high recurrence and mortality rates, even in young women. In an effort to improve outcomes for this subtype, gradually intensified neoadjuvant regimens have been evaluated, aiming to increase pathological complete response (pCR) rates and improve outcomes. Neoadjuvant therapy intensification through the addition of carboplatin and pembrolizumab to anthracycline-taxane chemotherapy has been approved as the standard of care. However, the intrinsic heterogeneity of TNBC renders the therapeutic response highly variable, and these results have not yet been validated in real-world studies. Aim of the study The objective of the study is to assess whether the benefit in terms of pCR achieved in clinical trials, through the intensification of neoadjuvant regimens with carboplatin and pembrolizumab is also confirmed in clinical practice. The secondary objective is to identify patient categories that obtain differential benefits from intensified regimens. Materials and Methods This is a multi-centric real-world study, conducted with a Target Trial Emulation (TTE) framework and real-world data of patients included in the international network GAMBIT. The study included patient with stage II/III TNBC and treated with one of the following neoadjuvant regimens in 22 European institutions: anthracycline and taxanes (AT), anthracycline, taxanes and carboplatin (AT-Cb), anthracycline, taxanes, carboplatin and pembrolizumab (AT-CB-pembro). We performed unadjusted analysis and adjusted analysis with Inverse Probability of Treatment Weighting (IPTW). The outcomes were the pCR rate (ypT0/is N0) and the Risk Difference in pCR rates in the comparisons of AT-Cb vs AT and AT-Cb-pembro vs AT-Cb. Results We included 1894 patients with TNBC, divided into three treatment cohorts as follows: 46.1% (n=873) in AT cohort, 16.5% (n=312) in AT-Cb cohort and 37.4% (n=709) in AT-Cb-pembro cohort. AT-Cb regimen demonstrated an improvement in the pCR – IPTW adjusted rates compared with AT regimen (49.6% vs 38.8%), with a Risk Difference of +10.7pp (95% CI, +3.4; +18.0; p=0.004). AT-Cb-pembro regimen demonstrated an improvement in the pCR – IPTW adjusted rates compared with AT-Cb regimen (64.6% vs 51.1%), with a Risk Difference of +13.6 pp (95% CI +8.5; +18.7; p<0.001). The analysis of pCR rates and the Risk Differences across the subgroups revealed a numerically heterogeneous effect in certain subgroups, such as a reduced benefit from the intensification with carboplatin in patients ≥65 years (RD -0.5%), stage III (RD 4.8%), or tumor-infiltrating lymphocytes (TILs) <30%, (RD -7.8%); or in contrast, a greater benefit in patients with ER-low tumors (ER 1-9%, RD 31.8%). Differently, we observed a greater benefit from a regimen with pembrolizumab in patients with stage III (RD 22.7%), ≥65 years (RD 31.2%) or TILs ≥30% (RD 18.8%). Conclusion The increase in pCR rates achieved in clinical trials through the intensification of neoadjuvant regimens with carboplatin and pembrolizumab is also confirmed in clinical practice. The analysis across subgroups documented a heterogeneous effect, allowing for the identification of clinicopathological factors with potential role in personalization of therapeutic regimens.

Nel tentativo di migliorare gli outcome di questo sottotipo, sono stati sperimentati regimi neoadiuvanti gradualmente intensificati con l’obiettivo di aumentare i tassi di risposte patologiche complete (pCR) e di migliorare gli outcome. L’intensificazione della terapia neoadiuvante con l’aggiunta di carboplatino e pembrolizumab alla chemioterapia con antracicline-taxani è stata approvata come standard terapeutica. Tuttavia, l’eterogeneità intrinseca del TNBC rende la risposta terapeutica estremamente variabile e questi risultati non sono stati validati in studi di real-world. Obiettivo L'obiettivo dello studio è verificare se il beneficio in termini di pCR dimostrato nei trial clinici, mediante l'intensificazione del regime neoadiuvante con carboplatino e con pembrolizumab, si confermi anche in pratica clinica. Obiettivo secondario è identificare categorie di pazienti che traggano un beneficio differenziale dall'intensificazione terapeutica. Materiali e Metodi Il presente è uno studio real-world multicentrico realizzato con un framework di Target Trial Emulation (TTE) e dati di pratica clinica di pazienti inclusi nello studio osservazionale internazionale GAMBIT. Sono state incluse pazienti affette da TNBC in stadio II-III trattate con uno dei seguenti regimi neoadiuvanti in 22 centri europei: antracicline e taxani (AT); antracicline, taxani, carboplatino (AT-Cb); antracicline, taxani, carboplatino, pembrolizumab (AT-Cb-pembro). Abbiamo condotto analisi non aggiustate e aggiustate mediante Inverse Probability of Treatment Weighting (IPTW). Gli outcome misurati sono stati il tasso di pCR (ypT0/is N0) e la Risk Difference nei tassi di pCR nel confronto AT-Cb vs AT e AT-Cb-pembro vs AT-Cb. Risultati Abbiamo incluso 1894 pazienti affette da TNBC divise come segue nelle tre coorti di trattamento: 46.1% (n=873) nella coorte AT, 16.5% (n=312) nella coorte AT-Cb, 37.4% (n=709) nella coorte AT-Cb-pembro. Il regime AT-Cb ha dimostrato un miglioramento nei tassi di pCR – IPTW aggiustata rispetto al regime AT (49.6% vs 38.8%) con una Risk Difference di +10.7pp (IC95%, +3.4; +18.0; p=0.004). Il regime AT-Cb-pembro ha dimostrato un incremento dei tassi di pCR – IPTW aggiustati rispetto al regime AT-Cb (64.6% vs 51.1%), con una Risk Difference i +13.6 pp (IC95%, +8.5; +18.7; p<0.001). L’analisi dei tassi di pCR e delle Risk Difference nei vari sottogruppi ha permesso di osservare un effetto numericamente eterogeneo in alcuni sottogruppi, come un minor beneficio dall’aggiunta di carboplatino in pazienti ≥65 anni (RD -0.5%), stadio III (RD 4.8%), o tumor-infiltrating lymphocytes (TILs) <30%, (RD -7.8%); o al contrario, un miglior beneficio nelle neoplasie con bassa espressione degli estrogeni (ER 1-9%, RD 31.8%). Diversamente, abbiamo osservato un miglior beneficio da un trattamento con pembrolizumab in pazienti con stadio III (RD 22.7%), ≥65 anni (RD 31.2%) o TILs ≥30% (RD 18.8%). Conclusioni L’aumento dei tassi di pCR, ottenuto nei trial clinici, mediante l’intensificazione del regime neoadiuvante con carboplatino e pembrolizumab, si conferma anche in pratica clinica. L’analisi per sottogruppi ha mostrato un effetto eterogeneo, permettendo di individuare fattori clinico-patologici con potenziale ruolo nella personalizzazione dei regimi terapeutici.
