Background. Diffuse Large B-Cell Lymphoma (DLBCL) is the most common subtype of aggressive non-Hodgkin lymphoma in adults. For more than two decades, R-CHOP (rituximab, cyclophosphamide, doxorubicin, vincristine, and prednisone) has represented the standard first-line treatment. The introduction of polatuzumab vedotin, an antibody-drug conjugate targeting CD79b, led to the development of the pola-R-CHP regimen (polatuzumab vedotin, rituximab, cyclophosphamide, doxorubicin, and prednisone), which demonstrated improved progression-free survival (PFS) compared with R-CHOP in the pivotal POLARIX trial. However, evidence from real-world clinical practice remains limited. Aim of the study. To compare the efficacy and safety of pola-R-CHP and R-CHOP in a real-world cohort of patients with DLBCL treated at the Hematology Unit of the Padua University Hospital. Materials and methods. A retrospective observational study was conducted on patients diagnosed with DLBCL and treated with first-line pola-R-CHP or R-CHOP. Data regarding patients’ clinical and biological characteristics, treatment exposure, therapeutic response, and adverse events were collected. Efficacy endpoints included treatment response, progression-free survival (PFS), disease-free survival (DFS), and overall survival (OS). Safety was assessed through the analysis of hematologic and non-hematologic toxicities associated with the two treatment regimens. Results. Analysis of treatment response showed a higher complete response (CR) rate in patients treated with pola-R-CHP compared with those receiving R-CHOP (90.5% vs 75.0%). No statistically significant differences were observed in terms of PFS and DFS, although a favorable trend in PFS was noted in the pola-R-CHP group. Regarding overall survival (OS), the difference observed between the two groups cannot be considered statistically robust due to the paucity of events and the absence of deaths in the pola-R-CHP group, factors that resulted in unstable estimates and limited the reliability of the analysis. Hematologic toxicities were the most frequent adverse events in both groups. The incidence of febrile neutropenia was numerically higher in the pola-R-CHP group (23.8% vs 6.3%), whereas neurotoxicity was broadly comparable between the two regimens (28.6% vs 20.0%). Age-stratified analysis showed that all episodes of febrile neutropenia occurred in patients aged ≥70 years, suggesting a relevant role of advanced age in increasing the risk of treatment-related infectious complications. Conclusions. In real-world clinical practice, the pola-R-CHP regimen demonstrated a favorable efficacy profile and manageable toxicity in patients with DLBCL. Although no statistically significant differences were observed for most efficacy endpoints, the higher complete response rate and the favorable trend in PFS suggest a potential clinical advantage over R-CHOP. Further studies with larger cohorts and longer follow-up are needed to confirm these findings.

Per oltre vent’anni il regime R-CHOP (rituximab, ciclofosfamide, doxorubicina, vincristina e prednisone) ha costituito lo standard terapeutico di prima linea. L’introduzione di polatuzumab vedotin, anticorpo-farmaco coniugato diretto contro il CD79b, ha portato allo sviluppo del regime pola-R-CHP (polatuzumab vedotin, rituximab, ciclofosfamide, doxorubicina e prednisone), che nello studio registrativo POLARIX ha dimostrato un miglioramento della progression-free survival (PFS) rispetto a R-CHOP. Tuttavia, le evidenze derivanti dalla pratica clinica reale sono ancora limitate. Scopo dello studio. Confrontare efficacia e sicurezza dei regimi pola-R-CHP e R-CHOP in una coorte real-world di pazienti affetti da DLBCL trattati presso l’U.O.C. di Ematologia dell’Azienda Ospedale-Università di Padova. Materiali e metodi. È stato condotto uno studio retrospettivo osservazionale su pazienti con diagnosi di DLBCL trattati in prima linea con pola-R-CHP o R-CHOP in cui sono state raccolte informazioni relative alle caratteristiche cliniche e biologiche dei pazienti, all’esposizione al trattamento, alla risposta terapeutica e agli eventi avversi. Gli endpoint di efficacia comprendevano la risposta al trattamento, la progression-free survival (PFS), la disease-free survival (DFS) e l’overall survival (OS). La sicurezza è stata valutata mediante l’analisi delle tossicità ematologiche e non ematologiche associate ai due regimi terapeutici. Risultati. L’analisi della risposta al trattamento ha evidenziato una percentuale di complete response (CR) numericamente superiore nei pazienti trattati con pola-R-CHP rispetto a quelli trattati con R-CHOP (90,5% vs 75,0%). Non sono emerse differenze statisticamente significative in termini di PFS e DFS, sebbene sia stato osservato un trend favorevole per la PFS nel gruppo pola-R-CHP. Per quanto riguarda l’OS, la differenza osservata tra i due gruppi non può essere considerata statisticamente robusta a causa dell’esiguo numero di eventi e dell’assenza di decessi nel gruppo pola-R-CHP, fattori che hanno determinato stime instabili e limitato l’affidabilità dell’analisi. Le tossicità ematologiche hanno rappresentato gli eventi avversi più frequenti in entrambi i gruppi. L’incidenza di neutropenia febbrile è risultata numericamente maggiore nel gruppo pola-R-CHP (23,8% vs 6,3%), mentre la neurotossicità è risultata sostanzialmente sovrapponibile tra i due regimi (28,6% vs 20,0%). L’analisi per fasce d’età ha evidenziato come gli episodi di neutropenia febbrile si siano verificati esclusivamente nei pazienti di età ≥70 anni, suggerendo un ruolo rilevante dell’età avanzata nel determinare il rischio di complicanze infettive correlate al trattamento. Conclusioni. Nella pratica clinica reale, il regime pola-R-CHP ha mostrato un profilo di efficacia favorevole e una tossicità complessivamente gestibile nei pazienti affetti da DLBCL. Sebbene non siano state osservate differenze statisticamente significative per la maggior parte degli endpoint di efficacia, il più elevato tasso di risposta completa e il trend favorevole per la PFS suggeriscono un potenziale vantaggio clinico rispetto a R-CHOP. Ulteriori studi con casistiche più ampie e follow-up più prolungati saranno necessari per confermare tali risultati.
