Background Primary cutaneous T-cell lymphomas (CTCLs) are the most common group of primary cutaneous lymphomas, and mycosis fungoides (MF) is their most frequent variant. The course is heterogeneous: generally indolent in the early skin limited stages, but potentially progressive. Biochemical and immunophenotypic markers such as the CD4/CD8 ratio, LDH, lymphocyte count and transaminases are used for staging and prognostic assessment, particularly in advanced disease; it remains unclear, however, whether an atypical or more severe cutaneous presentation at diagnosis is accompanied by corresponding systemic involvement. Aim of the study To assess, in patients with MF, whether an atypical or more severe cutaneous presentation, defined as a "complex phenotype" and suggestive of a potentially more aggressive clinical course, is associated, at diagnosis, with systemic biochemical and immunophenotypic alterations, with particular regard to the CD4/CD8 ratio. Materials and methods This was a single-center retrospective observational study of 24 patients with a clinical and histological diagnosis of MF, evaluated at the Dermatology Clinic of Padua Hospital. For each patient, demographic data, clinical phenotype at presentation and laboratory profile (LDH, absolute lymphocyte count, AST, ALT, CD4/CD8 ratio) were collected. The "complex phenotype" was defined by the presence of at least one of: folliculotropism, granulomatous slack skin, nodular/tumoral lesions, or (sub)erythrodermic disease. The primary endpoint was the presence of major biochemical alterations, defined as at least one of LDH > 450 U/L, lymphocytosis or transaminitis. The CD4/CD8 ratio was analyzed as an independent immunophenotypic parameter, using two thresholds: an "elevated" value (> 2.6) and one of "high clinical relevance" (≥ 10). Continuous variables were described non-parametrically; between-group comparisons were performed using Fisher's exact test and the Mann-Whitney U test (significance at p < 0.05). Results Of the 24 patients (12 male and 12 female; median age 61 years), 8 of the 21 evaluable for phenotype (38.1%) had a complex phenotype, and 8 were in a stage above IB, of whom 2 (9.5%) beyond stage II. The baseline profile was largely preserved (median LDH 234 U/L; median CD4/CD8 ratio 2.33). Major biochemical alterations were present in 5 of the 23 evaluable patients (21.7%) and an elevated CD4/CD8 ratio (> 2.6) in 11 of 23 (47.8%), with no value of high clinical relevance (≥ 10). No significant association emerged between the complex phenotype and major biochemical alterations (25.0% in patients with a complex phenotype and 23.1% in those without; p = 1.000), nor with an elevated CD4/CD8 ratio (37.5% and 50.0%; p = 0.670). Quantitative analysis of the individual parameters and stratification by stage likewise showed no significant differences. Conclusions In this predominantly early-stage cohort, the complexity of the cutaneous presentation was not associated with a biochemical or immunophenotypic profile indicative of a more aggressive clinical course, consistent with the indolent nature of early MF and with the predominant role of systemic markers in advanced disease. As a pilot study, these findings are exploratory and hypothesis-generating and require confirmation in larger, multicenter, prospective cohorts, correlating the cutaneous phenotype with the clinical course over time.

Marcatori ematochimici e immunofenotipici come il rapporto CD4/CD8, l'LDH, la conta linfocitaria e le transaminasi sono impiegati nella stadiazione e nella valutazione prognostica, soprattutto nelle forme avanzate; resta invece poco chiaro se una presentazione cutanea atipica o più severa alla diagnosi si accompagni a un corrispondente coinvolgimento sistemico. Scopo dello studio Verificare, in pazienti affetti da MF, se una presentazione cutanea atipica o più severa, definita come “fenotipo complesso” e indicativa di un decorso clinico potenzialmente più aggressivo, si associ, al momento della diagnosi, ad alterazioni sistemiche di tipo ematochimico e immunofenotipico, con particolare riguardo al rapporto CD4/CD8. Materiali e metodi Si tratta di uno studio osservazionale retrospettivo monocentrico su 24 pazienti con diagnosi clinica e istologica di MF, valutati presso la Clinica Dermatologica dell'Ospedale di Padova. Per ciascuno sono stati raccolti dati demografici, fenotipo clinico alla presentazione e profilo laboratoristico (LDH, conta linfocitaria assoluta, AST, ALT, rapporto CD4/CD8). Il “fenotipo complesso” è stato definito dalla presenza di almeno uno tra: follicolotropismo, granulomatous slack skin, lesioni nodulari/tumorali o quadro (sub)eritrodermico. L'endpoint principale era la presenza di alterazioni ematochimiche maggiori, definite dal riscontro di almeno una tra LDH > 450 U/L, linfocitosi o transaminite. Il rapporto CD4/CD8 è stato analizzato come parametro immunofenotipico indipendente, con due soglie: valore “elevato” (> 2,6) e “ad alta rilevanza clinica” (≥ 10). Le variabili continue sono state descritte in forma non parametrica; il confronto tra gruppi è stato condotto con il test esatto di Fisher e il test U di Mann-Whitney (significatività per p < 0,05). Risultati Dei 24 pazienti (12 maschi e 12 femmine; età mediana 61 anni), 8 dei 21 valutabili per fenotipo (38,1%) presentavano un fenotipo complesso e 8 erano in stadio superiore a IB, di cui 2 (9,5%) oltre lo stadio II. Il profilo basale è risultato complessivamente conservato (LDH mediano 234 U/L; rapporto CD4/CD8 mediano 2,33). Alterazioni ematochimiche maggiori erano presenti in 5 dei 23 pazienti valutabili (21,7%) e un rapporto CD4/CD8 elevato (> 2,6) in 11 su 23 (47,8%), senza alcun valore ad alta rilevanza clinica (≥ 10). Non è emersa alcuna associazione significativa tra fenotipo complesso e alterazioni ematochimiche maggiori (25,0% nei pazienti con fenotipo complesso e 23,1% in quelli senza; p = 1,000), né con il rapporto CD4/CD8 elevato (37,5% e 50,0%; p = 0,670). Anche l'analisi quantitativa dei singoli parametri e la stratificazione per stadio non hanno evidenziato differenze significative. Conclusioni In questa coorte, prevalentemente in stadio precoce, la complessità del quadro cutaneo alla presentazione non si è associata a un profilo ematochimico e immunofenotipico indicativo di un decorso clinico più aggressivo, coerentemente con la natura indolente della MF iniziale e con il ruolo dei marcatori sistemici prevalentemente nelle fasi più avanzate. Trattandosi di uno studio pilota, i risultati hanno valore esplorativo e di generazione di ipotesi e richiedono conferma in coorti più ampie, multicentriche e prospettiche, correlando il fenotipo cutaneo con il decorso clinico nel tempo.
