Meniscal tears of the knee are one of the most common orthopedic conditions and a frequent cause of pain, functional limitation, and premature joint degeneration. Due to the reduced vascularization of the meniscal tissue, especially in the inner areas, the ability to heal spontaneously is often limited, making the treatment of more extensive or degenerative tears complex. Traditional therapeutic strategies, such as partial meniscectomy or meniscal suturing, while providing short-term clinical benefits, may not be sufficient to fully restore the biomechanical function of the meniscus and prevent the progression to osteoarthritis. In light of these critical issues, in recent years, research in tissue engineering and biomaterials has assumed a central role in the development of innovative solutions for meniscal repair and regeneration. The biomaterials used in this field, including polymer scaffolds, hydrogels, biological matrices, and composite materials, are designed to promote cell proliferation, support tissue regeneration, and reproduce the mechanical and tribological properties of the native meniscus. Furthermore, these materials must ensure biocompatibility, adequate degradation over time, and integration with the surrounding tissue, without compromising knee joint stability. This paper will analyze the main anatomical and functional characteristics of the meniscus, the problems associated with meniscal tears, and current therapeutic options, with particular attention to the role of biomaterials in regenerative medicine. It will also describe the main biomaterials studied and/or used clinically, highlighting their mechanisms of action, advantages, limitations, and future prospects in the repair and regeneration of knee meniscal tears.

Le lesioni meniscali del ginocchio rappresentano una delle patologie ortopediche più diffuse e costituiscono una causa frequente di dolore, limitazione funzionale e degenerazione articolare precoce. A causa della ridotta vascolarizzazione del tessuto meniscale, soprattutto nelle aree interne, la capacità di guarigione spontanea risulta spesso limitata, rendendo complesso il trattamento delle lesioni più estese o degenerative. Le tradizionali strategie terapeutiche, quali meniscectomia parziale o sutura meniscale, pur garantendo benefici clinici nel breve termine, possono non essere sufficienti a ripristinare completamente la funzionalità biomeccanica del menisco e a prevenire l’evoluzione verso l’osteoartrosi. Alla luce di tali criticità, negli ultimi anni la ricerca nel campo dell’ingegneria tissutale e dei biomateriali ha assunto un ruolo centrale nello sviluppo di soluzioni innovative per la riparazione e la rigenerazione meniscale. I biomateriali impiegati in questo ambito, tra cui scaffold polimerici, idrogel, matrici biologiche e materiali compositi, sono progettati per favorire la proliferazione cellulare, sostenere la rigenerazione del tessuto e riprodurre le proprietà meccaniche e tribologiche del menisco nativo. Inoltre, tali materiali devono garantire biocompatibilità, adeguata degradazione nel tempo e integrazione con il tessuto circostante, senza compromettere la stabilità articolare del ginocchio. Il presente elaborato analizzerà le principali caratteristiche anatomiche e funzionali del menisco, le problematiche associate alle lesioni meniscali e le attuali opzioni terapeutiche, con particolare attenzione al ruolo dei biomateriali nella medicina rigenerativa. Verranno inoltre descritti i principali biomateriali studiati e/o utilizzati clinicamente, evidenziandone meccanismi d’azione, vantaggi, limiti e prospettive future nell’ambito della riparazione e rigenerazione delle lesioni meniscali del ginocchio.
