This thesis analyzes cannabidiol (CBD), one of the main phytocannabinoids found in Cannabis sativa, focusing on its therapeutic properties, pharmacokinetic characteristics, mechanisms of action in relation to the endocannabinoid system and other molecular targets, adverse events that may occur, and potential pharmacological interactions with other medications. Unlike tetrahydrocannabinol (THC), CBD does not have psychoactive effects, a characteristic that has favored its widespread use and scientific interest in recent years. The aim of this thesis is to review the existing literature to evaluate the efficacy and safety of CBD in the treatment of various clinical conditions, including epilepsy, anxiety disorders, chronic pain, inflammatory and neurodegenerative diseases, and other specific conditions. CBD-based medications and cosmetic formulations with their properties are then reported. The paper explores the current regulatory framework governing the procurement and dispensing of cannabidiol-based medicines, also highlighting the critical issues related to the regulation of CBD products. The findings demonstrate that CBD is a promising therapeutic substance, although further clinical studies are required to fully confirm its efficacy and define its long-term safety profile. Future research and clearer regulation will be crucial for its informed and widespread use.
Questo elaborato analizza il cannabidiolo (CBD), uno dei principali fitocannabinoidi presenti nella Cannabis sativa, soffermandosi sulle proprietà terapeutiche, sulle caratteristiche farmacocinetiche, sui meccanismi d’azione in relazione al sistema endocannabinoide e ad altri target molecolari, sugli eventi avversi che possono insorgere e sulle possibili interazioni farmacologiche con altri medicinali. A differenza del tetraidrocannabinolo (THC), il CBD non presenta effetti psicoattivi, caratteristica che ne ha favorito la diffusione e l’interesse scientifico negli ultimi anni. L’obiettivo della tesi è quello di esaminare la letteratura esistente al fine di valutare l’efficacia e la sicurezza del CBD nel trattamento di diverse condizioni cliniche, tra cui epilessia, disturbi d’ansia, dolore cronico, patologie infiammatorie, neurodegenerative e altri casi specifici. Vengono, quindi, riportati i medicinali a base di cannabidiolo e formulazioni cosmetiche con relative proprietà. L’elaborato approfondisce il quadro normativo vigente che regolamenta le modalità di approvvigionamento e dispensazione di medicinali a base di cannabidiolo, evidenziando anche le criticità legate alla regolamentazione dei prodotti a base di CBD. I risultati definiscono come il CBD rappresenti una sostanza promettente in ambito terapeutico, pur richiedendo ulteriori studi clinici per confermarne pienamente l’efficacia e definirne i profili di sicurezza a lungo termine. La ricerca futura e una regolamentazione più chiara saranno determinanti per il suo utilizzo consapevole e diffuso.
