Background. In transoral surgery for oral and oropharyngeal carcinoma, the adequacy of resection margins is one of the main prognostic determinants. The energy-based devices used for cutting and haemostasis produce lateral thermal injury that alters the tissue at the margin and may impair its histopathological readability. No ex vivo study on porcine tongue has compared the devices in current use for both cutting and coagulation while combining histological and thermal measurement. Aim of the study. The primary aim was to assess the feasibility and internal consistency of a morphometric protocol for quantifying thermal injury on an ex vivo porcine tongue model, applicable to both cutting and coagulation, integrating histological and thermographic measurement; given the pilot nature of the study, the comparison between devices was exclusively descriptive. Secondary aims: to compare, in a paired design, the extent of injury detectable on haematoxylin-eosin (HE) and Masson's trichrome (MT); to explore the relationship between thermographic parameters and the histological measurement; and to document the confounding introduced by ablative tissue loss. Materials and methods. Single-session, single-surgeon ex vivo pilot study on six fresh porcine tongues, three for each of the two independent arms. Cutting arm: cold-blade scalpel as negative control, ConMed System 5000 in monopolar mode, Vesalius Quantum Smart, Coblator II and Harmonic Focus+; coagulation arm: the same devices in coagulation mode, with untreated tissue as negative control, thermocautery as the reference condition for high superficial thermal exposure and the Winglobe Strom 2.0 in bipolar mode. Power settings were calibrated to obtain comparable incision depths; applications were recorded with a FLIR E90 thermal camera, from which maximum apparent temperature and the temperature-time area above 40 °C were obtained. For each device, one lesion was read with both stains. Injury depth was measured according to a zonal model, with five fixed-step grid measurements along the margin; in the coagulation arm total depth was reconstructed from the presumed original surface. The stain comparison was analysed with the exact Wilcoxon signed-rank test. Results. The protocol was completed on ten lesions and 209 measurements and passed all internal consistency checks: the ablation-zone components were additive (−3.4 % to +0.6 %) and negative controls showed no quantifiable changes. In the cutting arm, median depths on MT ranged from 99 µm (Vesalius Quantum Smart) to 410 µm (Harmonic Focus+), with a ranking matching that of the temperature-time area; in the coagulation arm they ranged from 268 µm (thermocautery) to 585 µm (Harmonic Focus+), and the thermographic ranking did not match the histological one. Reconstructed total depth was about 1.5-2.5 times the residual depth. Across the eight paired lesions, MT showed a greater extent of injury than HE in every case (estimated difference 174 µm, ratio 2.49, p = 0.008); in two lesions out of ten the injury boundary was not identifiable on HE. Conclusions. The protocol is feasible and internally consistent and provides the basis for sizing a definitive study. In the evaluable lesions, the estimated extent of injury depended on the stain used, with greater values on Masson's trichrome; for ablative devices, depth measured from the residual surface underestimates the overall effect. The comparison between thermography and histology is descriptive: rankings agreed in the cutting arm and did not agree in the coagulation arm. With one lesion per device, the comparison between devices remains exclusively descriptive and supports no safety ranking; the data generate hypotheses for adequately powered studies.

I dispositivi a energia impiegati per il taglio e l'emostasi producono un danno termico laterale che altera il tessuto al margine e può comprometterne la leggibilità istopatologica. Manca uno studio ex vivo su lingua suina che confronti i dispositivi di uso corrente nel taglio e nel coagulo affiancando misura istologica e termica. Scopo dello studio. L'obiettivo primario è valutare la fattibilità e la coerenza interna di un protocollo morfometrico per la quantificazione del danno termico su modello ex vivo di lingua suina, applicabile al taglio e al coagulo, integrando misura istologica e termografica; data la natura pilota dello studio, il confronto fra dispositivi è esclusivamente descrittivo. Obiettivi secondari: confrontare in disegno appaiato l'estensione del danno rilevabile su ematossilina-eosina (HE) e tricromica di Masson (MT); esplorare il rapporto fra parametri termografici e misura istologica; documentare il confondimento introdotto dalla perdita di sostanza da ablazione. Materiali e metodi. Studio pilota ex vivo, in sessione unica e con operatore unico, su sei lingue suine fresche, tre per ciascuno dei due bracci indipendenti. Nel braccio del taglio: bisturi a lama fredda come controllo negativo, ConMed System 5000 in monopolare, Vesalius Quantum Smart, Coblator II e Harmonic Focus+; nel braccio del coagulo gli stessi dispositivi in modalità di coagulo, con tessuto non trattato come controllo negativo, termocauterio come riferimento a elevata esposizione termica superficiale e Winglobe Strom 2.0 in bipolare. La potenza è stata tarata per ottenere profondità di incisione comparabili; le applicazioni sono state registrate con termocamera FLIR E90, ricavando la temperatura apparente massima e l'area temperatura-tempo sopra i 40 °C. Per ciascun dispositivo una lesione è stata letta in doppia colorazione. La profondità è stata misurata secondo un modello zonale, con cinque misure a griglia a passo fisso lungo il margine; nel coagulo la profondità totale è stata ricostruita dalla presumibile superficie originaria. Il confronto fra colorazioni si basa su test esatto di Wilcoxon per ranghi appaiati. Risultati. Il protocollo, completato su dieci lesioni e 209 misure, ha superato le verifiche interne di coerenza: le componenti della zona di ablazione sono risultate additive (scarto fra −3,4 % e +0,6 %) e i controlli negativi privi di alterazioni. Nel taglio le profondità mediane su MT sono comprese fra 99 µm (Vesalius Quantum Smart) e 410 µm (Harmonic Focus+), con ordinamento coincidente con quello dell'area temperatura-tempo; nel coagulo fra 268 µm (termocauterio) e 585 µm (Harmonic Focus+), e l'ordinamento termografico non coincide con quello istologico. La profondità totale ricostruita è pari a circa 1,5–2,5 volte quella residua. Sulle otto lesioni appaiate la MT ha restituito un'estensione del danno superiore alla HE in tutti i casi (differenza stimata 174 µm, rapporto 2,49, p = 0,008); in due lesioni su dieci il confine del danno non era identificabile su HE. Conclusioni. Il protocollo è applicabile e internamente coerente e fornisce la base per dimensionare uno studio definitivo. Nelle lesioni valutabili la stima dell'estensione del danno è dipesa dalla colorazione impiegata, con valori maggiori sulla tricromica di Masson; nei dispositivi ablativi la profondità misurata dalla superficie residua sottostima l'effetto complessivo. Il confronto fra termografia e istologia è descrittivo: gli ordinamenti concordano nel braccio del taglio e non in quello del coagulo. Con una lesione per dispositivo il confronto fra strumenti resta esclusivamente descrittivo e non consente graduatorie di sicurezza: i dati generano ipotesi per studi a numerosità adeguata.
