Pleural mesothelioma is a malignancy with low incidence but high mortality, predominantly affecting male patients in the sixth to seventh decade of life, with a history of occupational or environmental exposure to asbestos occurring 20-30 years before diagnosis. It is typically diagnosed at an advanced stage; however, in a minority of cases the diagnosis is made while the disease is still surgically resectable. In this latter case, the standard treatment in the neoadjuvant setting consists of chemotherapy based on pemetrexed and a platinum agent (carboplatin or cisplatin); more recently, the addition of immunotherapy in this setting has been investigated, on the basis of the results obtained in advanced-stage disease, although a direct comparison with chemotherapy alone is still lacking. The present study is designed as a case-control, retrospective-prospective study: its aim is to compare the efficacy of neoadjuvant chemo-immunotherapy versus chemotherapy alone (both followed by surgical pleurectomy/decortication) in terms of pathological response to treatment and survival. The primary objective is the evaluation of the pathological complete response (pCR) rate achieved with the two treatments; secondly, the major pathological response (MPR) rate is also assessed. The secondary objectives, instead, were to evaluate the clinical benefit in terms of event-free survival (EFS) and overall survival (OS), both calculated from the start date of neoadjuvant treatment. The present study compares two patient populations: the Real-World population, consisting of 32 patients treated at the Veneto Institute of Oncology (IOV) between 2017 and 2024, and the population of the CHIMERA clinical trial, comprising 39 patients enrolled in the eponymous trial between 2024 and 2026. Patients in the Real-World population received neoadjuvant chemotherapy according to the pemetrexed-platinum regimen, whereas patients in the CHIMERA population received pembrolizumab in addition to the same chemotherapy regimen. Statistical analyses of the collected data were performed using the chi-square test and Fisher's exact test for pCR and MPR; Kaplan-Meier curves, the log-rank test, and univariate and multivariate Cox analysis for EFS and OS; propensity score matching (PSM) was additionally performed on pCR. The results of these analyses showed no statistically significant differences for any endpoint (pCR: OR=8,24, p=0,122; MPR: OR=2,09, p=0,125/0,203; EFS: HR=1,46, p=0,348; OS: p=0,063). By contrast, PSM on pCR showed a statistically significant difference (OR=1,07; 95% CI 1,00-1,15; p=0,045), with a 6,9% increase in the probability of achieving pCR in the CHIMERA population. It is worth emphasizing the marked asymmetry in the maximum follow-up for OS (95,3 months in the real-world population vs. 18,6 months in the CHIMERA population), which represents one of the main limitations of the study and of the analyses performed, together with the small sample size and the limited number of observed events. Overall, the present study suggests a possible benefit of neoadjuvant immunotherapy in surgically resectable pleural mesothelioma, with the strongest signal observed for pCR (statistically significant after PSM); survival endpoints, however, remain inconclusive due to a still short and immature follow-up, and a survival benefit associated with immunotherapy cannot therefore be ruled out at this time. Further randomized studies, conducted on larger cohorts and with a longer and more complete follow-up, are needed to confirm these preliminary findings.

Il mesotelioma pleurico è una neoplasia a bassa incidenza ed elevata mortalità che colpisce prevalentemente i soggetti di sesso maschile nella VI-VII decade di vita, con una storia di esposizione occupazionale o ambientale all’asbesto nei 20-30 anni precedenti alla diagnosi. Si tratta di una neoplasia tipicamente diagnosticata in stadio avanzato; tuttavia, in una minoranza di casi la diagnosi viene posta quando la malattia si trova ancora in uno stadio chirurgicamente resecabile. In quest’ultimo caso il trattamento standard prevede nel setting neoadiuvante una chemioterapia a base di pemetrexed e derivati del platino; recentemente, è stata studiata l’aggiunta dell’immunoterapia in questo setting, sulla scorta dei risultati ottenuti negli stadi avanzati, sebbene manchi ancora un confronto diretto con la sola chemioterapia. Il presente studio si configura come uno studio caso-controllo, retrospettivo-prospettico: l’obiettivo del presente studio è confrontare l’efficacia del trattamento neoadiuvante basato sulla chemio-immunoterapia rispetto alla sola chemioterapia (entrambi seguiti dall’intervento chirurgico di pleurectomia/decorticazione), in termini di risposta patologica al trattamento e di sopravvivenza. L’obiettivo primario è infatti la valutazione del tasso di risposta patologica completa (pCR) ottenuto a seguito dei due trattamenti; in secondo luogo, viene valutato anche il tasso di risposta patologica maggiore (MPR). Gli obiettivi secondari sono rappresentati, invece, dalla valutazione del beneficio in termini di sopravvivenza libera da eventi (EFS) e di sopravvivenza globale (OS), calcolate a partire dalla data di inizio del trattamento neoadiuvante. Il presente studio confronta due popolazioni di pazienti: la popolazione Real World, costituita da 32 pazienti trattati presso l’Istituto Oncologico Veneto (IOV) tra il 2017 e il 2024, e la popolazione dello studio clinico CHIMERA, composta da 39 pazienti arruolati all’interno dell’omonimo studio clinico tra il 2024 ed il 2026. I pazienti della popolazione Real World sono stati trattati con chemioterapia neoadiuvante secondo lo schema pemetrexed-derivati del platino, mentre i pazienti della popolazione CHIMERA sono stati trattati con pembrolizumab in aggiunta al medesimo schema di chemioterapia. Le analisi statistiche sui dati raccolti sono state condotte con test del chi-quadrato e test esatto di Fisher per pCR e MPR; curve di Kaplan-Meier, log-rank test e analisi di Cox uni- e multivariate per EFS e OS; su pCR è stato inoltre eseguito un Propensity Score Matching (PSM). I risultati di tali analisi non hanno mostrato differenze statisticamente significative per nessun endpoint (pCR: OR=8,24, p=0,122; MPR: OR=2,09, p=0,125/0,203; EFS: HR=1,46, p=0,348; OS: p=0,063). Il PSM su pCR ha invece mostrato una differenza statisticamente significativa (OR=1,07; IC 95% 1,00-1,15; p=0,045), con un incremento del 6,9% della probabilità di ottenere pCR nella popolazione CHIMERA. Va sottolineata la marcata asimmetria nel follow-up massimo per OS (95,3 mesi Real World vs 18,6 mesi CHIMERA), che rappresenta uno dei limiti principali dello studio e delle analisi svolte, oltre all’esiguità del campione e degli eventi osservati. Complessivamente, il presente studio suggerisce un possibile beneficio dell’immunoterapia neoadiuvante nel mesotelioma pleurico chirurgicamente resecabile, con il segnale più solido osservato per la pCR (statisticamente significativo dopo PSM); gli endpoint di sopravvivenza restano invece non conclusivi, a causa del follow-up ancora breve ed incompleto: al momento non è possibile escludere quindi la presenza di un beneficio in termini di sopravvivenza associato all’immunoterapia. Sono quindi necessari ulteriori studi randomizzati, condotti su casistiche più ampie e con un follow-up più lungo e completo per poter confermare questi risultati preliminari.
