Idiopathic Vestibular Syndrome (IVS) in dogs is frequently associated with ipsilateral Facial Neuropathy (FN), representing the clinical picture recently defined in the literature as Facial and Vestibular Neuropathy of Unknown Origin (FVNUO). However, it is hypothesized that the actual involvement of the facial nerve (CN VII) is underestimated, as it can manifest subclinically, thus escaping detection during a standard neurological examination. Instrumental identification of potential subclinical forms is also hindered by the known technical limitations of Magnetic Resonance Imaging (MRI) in detecting contrast enhancement of CN VII. Therefore, the primary objective of this retrospective study is to validate the quantitative evaluation of denervation hypotrophy of the caudal belly of the digastric muscle (CBDM) via MRI as an indicator to reveal the true incidence of concomitant subclinical ipsilateral FN in dogs diagnosed with IVS. The study included 81 dogs diagnosed with IVS between 2020 and 2025. For each patient, the Cross-Sectional Area (CSA) of the right and left CBDM was measured on T2-weighted transverse MRI sequences. The relative percentage volumetric variation between the antimeres was calculated, establishing a 5% threshold to define relevant atrophy. The population was subsequently stratified based on the extent of the hypotrophy, its lateralization compared to the vestibular signs, and the presence of overt clinical manifestations of facial paresis. Data analysis revealed that 51 out of 81 subjects (63%) presented a CBDM atrophy ≥ 5%. Within this group, 64,7% (33 dogs) showed hypotrophy ipsilateral to the IVS, confirming a statistically significant topographic association (p = 0.049). Only 22 patients in the entire sample showed clinical signs referable to FN. Consequently, in 31 cases, the CN VII affection was identified solely through morphometric investigation of the CBDM, thus proving to be subclinical. Furthermore, in the group with relevant ipsilateral atrophy (≥ 5%), a statistically significant correlation (p = 0.001) emerged between the time elapsed since the onset of IVS and the extent of the volumetric reduction, supporting the hypothesis that the acute vestibular symptomatology represents the concurrent evolution of a pre-existing pathological process affecting CN VII. In conclusion, the study demonstrates that the incidence of cranial polyneuropathy (FVNUO) is markedly higher than estimated by clinical investigation alone. CBDM CSA measurement is confirmed as a structural indicator of good diagnostic utility for unmasking clinically inapparent forms of FN. The obtained evidence suggests a re-evaluation of the clinical approach to IVS with a more thorough investigation of the facial region, highlighting the need to perfect instrumental methods aimed at identifying CN VII enhancement via MRI, and laying the foundations for the diagnostic implementation of electromyography to overcome methodological limits in the evaluation of potential bilateral FNs.

Abstract

Idiopathic Vestibular Syndrome (IVS) in dogs is frequently associated with ipsilateral Facial Neuropathy (FN), representing the clinical picture recently defined in the literature as Facial and Vestibular Neuropathy of Unknown Origin (FVNUO). However, it is hypothesized that the actual involvement of the facial nerve (CN VII) is underestimated, as it can manifest subclinically, thus escaping detection during a standard neurological examination. Facial Neuropathy in Dogs with Ipsilateral Idiopathic Peripheral Vestibular Syndrome: Is Atrophy of the Caudal Belly of the Muscle Digastricus a Sign of Subclinical Disease? A Retrospective Study on Magnetic Resonance Imaging.
