Introduction: The dorsal atlantoaxial band is a compressive lesion located dorsally between the atlas and axis, identifiable on sagittal T2-weighted MRI as reduced or interrupted cerebrospinal fluid signal in the dorsal subarachnoid space. Objective: To assess the association between band presence and grade and age, sex, breed, foramen magnum height and patient position during MRI. Materials and methods: A retrospective observational study was conducted on 51 dogs, including 38 Chihuahuas and 13 Cavalier King Charles Spaniels, which underwent MRI between January 2023 and August 2024. The band was classified using a four-grade semiquantitative scale, ranging from 0 to 3. Age, sex, breed, patient position during the examination and foramen magnum height were recorded. Categorical variables were analysed using Fisher’s exact test and the Mann–Whitney U test, while foramen magnum height was assessed using Spearman’s rank correlation coefficient and the Kruskal–Wallis test. Statistical significance was set at p<0.05. Results: Of the 51 subjects, 18 were classified as grade 0, 13 as grade 1, 7 as grade 2 and 13 as grade 3. No significant associations were found between band grade and age (p=0.660), sex (p=0.187) or breed (p=0.738). A significant association was observed between patient position during the examination and band grade (p=0.004). Subjects examined in the prone position were more frequently classified as having higher band grades, whereas those examined in the supine position were more frequently classified as grade 0. The comparison of the band score between the two positions was also statistically significant (p=0.001). Foramen magnum height showed a weak, non-significant negative correlation with band grade (r=-0.25; p=0.080); the comparison among the four grades was not statistically significant (p=0.357). Conclusions: In the sample analysed, patient position during the MRI examination was the only variable associated with atlantoaxial band grade. However, give that every patient only presented a single RM study either in a prone or supine position, a causal relationship between patient position and band grade could not be established.
