Introduction: the onset of a neoplasm in the first years of life and early hospitalization expose the child to risk factors that may interfere with language emergence. Although recent scientific evidence highlights the need for timely speech language monitoring, clinical investigations show that speech- language therapy intervention often occurs with considerable delay relative to diagnosis (Chami et al., 2022; Docking et al., 2025). The characterization of the communicative-linguistic profile at disease onset appears essential to overcome this critical issue and to guide early rehabilitative support pathways already in the acute phase. Objectives: to examine the communicative-linguistic profile in a cohort of pediatric patients assesses at disease onset, describing communicative-linguistic abilities, particularly the differences between comprehension and production, in order to identify potential discrepancies between these two components and to investigate possible differences related to gender and/or pathology type. Materials and Methods: the study involved a sample of 30 children (16 males, 14 females) aged between 2 and 37 months (M=21.1; SD=9.5) hospitalized in the Pediatric Oncohematology Unitof the Padova University Hospital, recruited as part of the project "Stai bene 4.0", promoted by AIL. The assessment was conducted using direct observation tools targeting different linguistic levels: lexical (PinG), morphosyntactic (PCGO, GALS), articulatory (BAMF), and cognitive (Griffiths). Statistical analysis was performed using non-parametric tests via Jamovi software. Results: the analysis revealed a statistically significant difference (p<0.01) between the well- preserved verbal comprehension, which was largely within normal range at both the lexical and morphosyntactic levels (PCGO: 85.7% of patients within normal range), and a severe decline in verbal production (PinG: over 54% of patients at the 5th percentile), accompanied by a delay in sentence structures (GALS: mean: 1.83) and in articulatory control (BAMF mean: 3.77). This profile of expressive fragility proved to be uniform and independent of gender and type of oncological disease(p>0.05), correlating selectively with the communicative score of the Griffiths III scales (p<0.05). Conclusion: the communicative-linguistic profile that emerged, characterized by impaired verbal production alongside preserved comprehension, confirms the importanceof early speech and language therapy assessment at disease onset to promptly identify at- risk children and guide rehabilitative interventions already in the acute phase.
