ENGLISH ABSTRACT Introduction
Periodontal health is a cornerstone for every kind of patient and its maintenance is still an important challenge for dentistry today. In recent years, several new treatment approaches for the treatment of periodontitis have developed, always in association with conventional root planing and scaling therapy (SRP). Among these we can include a marked increase in the use of different types of lasers, both as antibacterial photodynamic therapy (aPDT), and for catalyzing tissue healing after periodontal treatment thanks to low-level laser therapy (LLLT). Purpose
The thesis aims to evaluate which lasers are the most used in periodontal treatment, as well as verify the effectiveness of aPDT and LLLT therapies in inflammatory processes of soft and hard tissues. Materials and methods
The study was divided into three macro-topics. The first two are descriptive: in the first it was introduced what the laser is, its functioning and its main uses in medical disciplines, while in the second the periodontal disease, its causes and its consequences were illustrated. The third macro-topic includes scientific studies, all researched in electronic literature, in particular in the journals 'Pub Med', 'Embase' and 'Cochrane' and published online from January 2017 until December 2021. Only randomized controlled studies were considered. RCT and meta-analysis. Conclusion Studies have confirmed that SRP scaling and root planing is the most effective treatment for periodontitis. APDT-assisted therapy improves the decontamination of areas that are more difficult to access with manual and ultrasonic instruments and improves periodontal indices such as the PD probing depth (> 1mm compared to SRP monotherapy) and the clinical attachment level CAL (> 1mm compared to monotherapy SRP). LLLT therapy optimizes tissue healing after periodontal treatment thanks to effective fibroblastic stimulation. In the medium to long term, however, the effectiveness of lasers in periodontology is still unclear. To improve this study model too, it will be necessary to draw up standardized protocols to facilitate future evaluation.
