Haemorrhoidal disease is the most frequently diagnosticated benign anorectal illness, although its pathophysiology is still unknown. There are different treatments for every degree of Goligher’s classification. Second and third degree haemorrhoids are mainly treated with rubber band ligation (RBL). This technique is safe, effective and low-cost. Pain and bleeding are the most common complications. Results 84 RBL sessions were included in the study for 77 patients. The majority of patients suffered pain the day of the RBL (slight pain in 14,3%, moderate pain in 11,9%, severe pain in 14,3%, total pain was 40,5%) and the day after (slight pain in 13,1%, moderate pain in 21,4%, severe pain in 7,1%, total pain was 41,7%), on day 3 only 19% of patients experienced pain (only 2,4% had severe pain), the following days of the week pain was at 10,8% (1,2% of patients had severe pain), it then remained stable during the second and fourth week (no one had severe pain). An older age resulted to be a weak protective factor for pain on the third day (OR 0,959, CI 95% 0,921-0,997, p=0,0370) and the lateral left positioning of the band (3 o’clock) a mild risk factor for pain from the fourth to the seventh day (OR 5,750, CI 95% 1,115-29,648, p=0,0366). The number of bands did not appear to be a risk factor for pain, but the p-value was near the statistical significance(p=0,0747). The use of oral analgesia was higher in the first two days, on the first day was 31%, on the second 37%. The anterior right positioning of the band (11 o’clock) appeared to be a strong risk factor for oral analgesia intake on the day of the procedure (p=0,0101, OR 9,952, CI 95% 1,248-79,347), and a mild risk factor for the day after (p=0,0228, OR 3,772, CI 95% 1,147-12,408). Bleeding occurred mainly in the first week after RBL, it concerned the 33,3% of the patients, while on the second and the fourth week bleeding was present in the 17,8% and 18% of patients. Anti-platelet drugs were not a risk factor for bleeding, but p-value was near the statistical significance on the second week (p=0,0860). Pruritus did not variate much in the month of follow-up, it was described by 12-15,1% of patients. Mesalazine suppositories resulted to be a mild risk factor for pruritus the second week after RBL (p=0,0320, OR=5,833, CI 95% 1,163-29,251).

La malattia emorroidaria è la condizione patologica ano-rettale benigna più frequentemente diagnosticata, ciò nonostante la sua patogenesi resta ancora sconosciuta. A seconda del grado della classificazione di Goligher i pazienti sono indirizzati verso diverse strategie terapeutiche. Le emorroidi di secondo e terzo grado vengono principalmente trattate con legatura elastica. Questa tecnica è sicura, efficace ed economica. Le principali complicanze sono il dolore e il sanguinamento. Risultati Lo studio ha compreso 84 sedute di legatura elastica emorroidaria per 77 pazienti. La maggior parte dei pazienti ha presentato dolore il giorno della legatura (di grado lieve nel 14,3%, moderato nel 11,9%, severo nel 14,3%, totale 40,5%) e il giorno successivo (di grado lieve nel 13,1%, moderato nel 21,4%, severo nel 7,1%, totale nel 41,7%), il terzo giorno la quota di pazienti con dolore si è ridotta al 19% (severo solo nel 2,4%) e il resto della prima settimana è arrivata al 10,8% (severo nell’ 1,2%), poi restando stabile anche la seconda e quarta settimana (nessun dolore severo alla seconda settimana). Tra i fattori di rischio considerati è risultato che una maggiore età è un fattore protettivo debole per il dolore al terzo giorno (OR 0,959, CI 95% 0,921-0,997, p=0,0370) e una legatura in sede laterale sinistra (ore 3) è un fattore di rischio moderato per il dolore tra il quarto e il settimo giorno dalla legatura (OR 5,750, CI 95% 1,115-29,648, p=0,0366). Il numero di lacci applicati non è risultato essere un fattore di rischio significativo per il dolore tra il quarto e il settimo giorno, ma era vicino alla significatività statistica (p=0,0747). L’utilizzo della terapia analgesica è stato maggiore nei primi due giorni, il primo giorno l’ha assunta il 31%, il secondo il 37%. La legatura in sede anteriore destra (ore 11) è risultata essere un fattore di rischio forte per l’assunzione di terapia analgesica orale il giorno della legatura (p=0,0101, OR 9,952, CI 95% 1,248-79,347), e moderato per il giorno successivo alla legatura (p=0,0228, OR 3,772, CI 95% 1,147-12,408). Il sanguinamento è avvenuto soprattutto nella prima settimana dopo la legatura in cui è stato riferito nel 33,3% dei pazienti, mentre nel 17,8% e nel 18% dei pazienti la seconda e la quarta settimana. La terapia antiaggregante non è risultata essere un fattore di rischio significativo per il sanguinamento, ma il p-value era vicino alla significatività la seconda settimana (p=0,0860). Il prurito è rimasto pressoché costante tra il 12% e il 15,1% nel mese di follow-up. Gli antinfiammatori topici (Mesalazina supposte) sono risultati essere un fattore di rischio moderato per il prurito alla seconda settimana dopo legatura (p=0,0320, OR=5,833, CI 95% 1,163-29,251).
