ABSTRACT BACKGROUND Primary aldosteronism (PA) is the most frequent cause of secondary hypertension and compared to the so-called “essential” hypertension, it is associated with an excess risk of target organ damage and major cardio-reno-vascular events. Till now, the information available on the effect of aldosterone on ocular vascularization is limited to the study of the diameters of the retinal arterioles and venules through fundoscopy. AIM OF THE STUDY To prospectively evaluate the effects of medical and surgical treatment of primary aldosteronism on the microvascularization of the retina and choroid, focusing on the superficial and deep retinal plexuses and on the choriocapillary layer. MATERIALS AND METHODS We enrolled 11 patients referred to the Arterial Hypertension Center of the Padua Hospital between May 2021 and June 2022, with a diagnosis of unilateral PA. Using a prospective within-patient design, each patient was examined three times: at screening (in washout from RAAS confounding drugs, WO), after  1 month of MRA therapy (MRA) and  3 months after adrenalectomy (PO). At each timepoint, an eye examination was performed at the Ophthalmology Operating Unit of S. Antony Hospital. OCT and OCTA were performed, through the use of OCT Solix FullRange (Optovue, Inc., Freemont, CA, USA), acquiring transversal 2D scans of 3x3mm, centered on the fovea, from which we extrapolated the en face images of the superficial and deep retinal plexuses and of the choriocapillary layer - by means of automatic segmentation operated by the instrument software and the subsequent evaluation by the operator -. The images were then binarized and skeletonized using the opensource ImageJ software. From these, the following quantitative parameters were quantified: the vascular density (VAD), the number of vessels (VLF) and the avascular area of the fovea (FAZ). Linear scans, centered on the fovea, high density (HD) at 180 °, 6 mm length, were also acquired, on which the thicknesses of the choroid were measured in different points, first by the instrument, then by reviewing the images to confirm the correct references. RESULTS After adrenalectomy, aldosteronism was biochemically cured in all patients (except one, probably suffering from bilateral form). Arterial hypertension was cured in 6 patients and improved in 4 patients who needed less drugs to achieve significantly blood pressure control. In all linear measurements relating to choroidal thickness, we found a significant decrease in the thickness of the choroid, between screening (WO) and PO. In the superficial plexus, VADs and VLFs decreased (p = 0.016 and p = 0.0330), between screening (WO) and PO. VADp decreased in the deep retinal plexus (p = 0.0227). No quantitative parameter changed significantly with the MRA treatment in the choriocapillary layer. FAZ remained unchanged after surgery and drug MRA administration. CONCLUSIONS This study demonstrates the feasibility of measuring in a direct and non-invasive way, through OCT and OCTA, the choroidal thickness and the density and number of vessels in the superficial and deep layers of the retina. This will allow understanding the alterations induced by primary aldosteronism, the paradigm of salt-volume dependent hypertension. These conclusions can open the way for the systematic study of chorioretinal alterations induced by systemic hypertension.

RIASSUNTO BACKGROUND L’aldosteronismo primario (PA) è la più frequente causa di ipertensione arteriosa secondaria. Rispetto all’ipertensione cosiddetta “essenziale”, si caratterizza per un eccesso di rischio di danno d’organo e di eventi cardio-reno vascolari maggiori. Finora le informazioni disponibili sull’effetto dell’aldosterone a livello della vascolarizzazione oculare, sono limitate allo studio dei diametri delle arteriole e venule retiniche tramite fundoscopia. SCOPO DELLO STUDIO Valutare prospetticamente gli effetti del trattamento farmacologico e della guarigione chirurgica dell’aldosteronismo primario e della sua correzione sulla microvascolarizzazione della retina e della coroide, in particolare sui plessi retinici superficiale e profondo e sullo strato coriocapillare, nei pazienti trattati medicalmente con MRA e chirurgicamente. MATERIALI E METODI Lo studio ha arruolato 11 pazienti riferiti al Centro dell’Ipertensione Arteriosa dell’Azienda Ospedaliera di Padova tra maggio 2021 e giugno 2022, con diagnosi di PA ed evidenza di lateralizzazione all’AVS. Secondo un disegno prospettico intra-paziente ogni paziente è estato esaminato tre volte: allo screening in washout da farmaci confondenti il RAAS (WO), dopo  1 mese di terapia con MRA (MRA) e  3 mesi dopo la surrenectomia (PO). Ad ogni timepoint, è stata eseguita una visita oculistica, presso l’ambulatorio dell’Unità Operativa di Oculistica. Sono stati eseguiti OCT e OCTA, tramite l’utilizzo dell’OCT Solix FullRange (Optovue, Inc., Freemont, CA, USA), acquisendo scansioni 2D trasversali di 3x3mm, centrate sulla fovea da cui abbiamo estrapolato le immagini en face dei plessi retinici superficiale e profondo e dello strato coriocapillare -tramite segmentazione automatica operata dal software dello strumento e successiva valutazione dell’operatore-. Le immagini sono poi state binarizzate e scheletonizzate tramite software opensource ImageJ. A partire da queste, sono stati quantificati i seguenti parametri quantitativi: VAD, VLD e FAZ che stimano, rispettivamente, la densità vascolare, la numerosità di vasi e l’area avascolare della fovea. Sono state acquisite anche scansioni lineari, centrate sulla fovea, ad elevata densità (HD) a 180° di 6 mm di lunghezza, su cui sono stati misurati gli spessori della coroide in diversi punti, prima da parte dello strumento, poi tramite revisione delle immagini per conferma dei corretti reperi. RISULTATI Dopo surrenectomia, l’aldosteronismo è guarito biochimicamente in tutti i pazienti (tranne uno, verosimilmente affetto da forma bilaterale), inoltre si è osservata la cura dell’ipertensione in 6 pazienti e il miglioramento della stessa in 4 pazienti, che devono assumere alcuni farmaci per ottenere il controllo pressorio. In tutte le misure lineari relative agli spessori coroideali si è dimostrato esserci una significativa diminuzione dello spessore della coroide, tra screening (WO) e PO. Nel plesso superficiale VADs e VLFs sono diminuiti (p=0.016 e p=0.0330), tra screening (WO) e PO. Nel plesso retinico profondo è diminuito il VADp (p=0.0227). Nessun parametro quantitativo si è modificato significativamente nello strato coriocapillare e con l’impiego di MRA. La FAZ non ha mostrato cambiamenti significativi, né dopo chirurgia né dopo assunzione di MRA. CONCLUSIONI Questo studio dimostra la fattibilità di misurare in modo diretto e non invasivo, tramite OCT e OCTA, gli spessori coroideali, la densità e il numero dei vasi negli strati superficiale e profondo della retina. Ciò ha consentito di comprendere le modificazioni indotte dall’aldosteronismo primario, il paradigma dell’ipertensione sale-volume dipendente. Queste considerazioni finali possono aprire la strada allo studio sistematico delle alterazioni corioretiniche indotte dall’ipertensione sistemica.
