Epilepsy is a neurological disorder characterized by a predisposition to the onset of epileptic seizures. Pediatric epilepsies are often characterized by phenotypic heterogeneity and a broad spectrum of comorbidities, such as intellectual disability and psychomotor delay, autism spectrum disorder (ASD) with attention deficit hyperactivity disorder (ADHD), learning disabilities, language delay and movement disorders. The aim of the study was the identification in a large cohort of affected subjects, referred by Italian Pediatric Neurology Centers, of genetic bases of early-onset pediatric epilepsies and of the most prevalent comorbidities, in order to establish better genotype-phenotype correlations. Clinical information were collected by the referring physicians by completing preset clinical questionnaires. Genomic DNA of 361 individuals was analyzed by targeted Next Generation Sequencing (NGS) using two custom panels containing respectively 31 genes (EIEE-Early onset infantile Encephalopathies) and 93 genes (EPI-Pediatric Epilepsies), selected as the most important known or putative early epilepsy-genes. The identified variants were validated and segregated using the classical Sanger sequencing method. By reviewing the available clinical information, we detected the following comorbidities in our epileptic cohort: intellectual disability/psychomotor delay (49.58%), language delay (36.77%), behavioral disorders (21.88%), movement disorders (9.42%) and learning disabilities (6.93%). Overall, a definite molecular diagnosis was obtained in 56 individuals (15.51%). Taking into consideration the age of seizure onset, the highest diagnostic yield was obtained in the neonatal period (0-1 month; 27.59%), followed by infancy (2-24 months; 24.83%), middle childhood (6-11 years; 10%) and early childhood (4-5 years; 7.50%). A total of 73 variants were identified in 56 individuals; 60 variants were classified as Likely Pathogenic/Pathogenic (LP/P), while 13 VUS variants were found to be extremely rare and inherited from reportedly asymptomatic parents; among the 60 LP/P variants, 26 were found to be novel and 34 resulted to be de novo. In this thesis we reported the clinical characterizations of subjects with early-onset epilepsy, in whom were detected novel pathogenic variants in genes encoding voltage-dependent ion channels found to be prevalently altered, such as SCN1A, SCN2A, SCN8A, KCNQ2, and in the HCN1 gene as a relevant example of phenotypic heterogeneity. An integrated evaluation of neurodevelopmental comorbidities was performed in order to make better genotype-phenotype correlations. The data we generated support the high specificity of our genetic panels for the diagnosis of early-onset epilepsy. The frequency of neurobehavioral and neurocognitive comorbidities among the individuals with epilepsy of our cohort, further reinforces the importance of including neurodevelopmental epilepsy in the NDDs family. An early and accurate molecular diagnosis allows to plan a personalized multidisciplinary care pathway and to get access to targeted therapeutic treatments and to appropriate genetic counselling.

L’epilessia è un disturbo neurologico caratterizzato dalla predisposizione all’insorgenza di crisi epilettiche. Le epilessie dell'età evolutiva sono spesso caratterizzate da ampia eterogeneità fenotipica e frequentemente associate ad uno spettro di comorbidità che include disabilità intellettiva (ID) e ritardo psicomotorio, disturbo dello spettro autistico (ASD) con disturbo da deficit d’attenzione/iperattività (ADHD), disturbi dell’apprendimento, del linguaggio e del movimento. Lo scopo dello studio è stato quello di identificare le basi genetiche responsabili delle epilessie dell’età evolutiva in un’ampia casistica di soggetti affetti, riferiti da Centri di Neurologia Pediatrica del territorio Italiano, e di individuare le comorbidità maggiormente rappresentate nella nostra coorte, in modo da poter effettuare migliori correlazioni genotipo-fenotipo. Le informazioni cliniche sono state raccolte dai medici di riferimento tramite la compilazione di schede preimpostate. Il DNA genomico di 361 individui è stato sequenziato mediante targeted Next Generation Sequencing (NGS) utilizzando due pannelli custom contenenti rispettivamente 31 geni (EIEE-Encefalopatie epilettiche/Epilessie precoci) e 93 geni (EPI-Epilessie dell'età evolutiva) noti o fortemente indiziati come geni-malattia per epilessia dell’età evolutiva. Le varianti identificate sono state validate e segregate mediante metodo classico di sequenziamento Sanger. La revisione delle informazioni cliniche della nostra coorte di soggetti affetti da epilessia ha riscontrato le seguenti comorbidità: ritardo psicomotorio/disabilità intellettiva (49.58%), disturbo del linguaggio (36.77%), disturbo del comportamento/ASD (21.88%), disturbo del movimento (9.42%) e disturbi dell’apprendimento (6.93%). Considerando la popolazione in esame nel suo complesso, è stato possibile ottenere una diagnosi molecolare certa in 56 individui (15.51%). Considerando l’età all’esordio delle crisi epilettiche, la più alta resa diagnostica è stata ottenuta nell’età neonatale (0-1 mese; 32.35%), seguita da quella infantile (2-24 mesi; 18.99%), in età 6-11 anni (9.52%) e in età 3-5 anni (6.15%). Un totale di 73 varianti è stato identificato in 56 individui; 60 varianti sono state classificate come Likely Pathogenic/Pathogenic (LP/P), mentre 13 varianti di incerto significato sono risultate essere estremamente rare e trasmesse da genitori riferiti come asintomatici. Tra le 60 LP/P, 26 sono varianti nuove e 34 de novo. In questo lavoro di tesi sono state in particolare riportate le caratterizzazioni cliniche di soggetti con epilessia precoce, che presentavano varianti patogeniche nuove in geni codificanti canali ionici voltaggio-dipendenti più frequentemente mutati nella nostra casistica, quali SCN1A, SCN2A, SCN8A, KCNQ2, e nel gene HCN1 come esempio particolarmente rilevante di eterogeneità fenotipica. Inoltre, sono state integrate le informazioni cliniche sulle comorbidità associate alle patologie del neurosviluppo (NDDs), nell'ottica di effettuare migliori correlazioni genotipo-fenotipo. I dati generati supportano l’elevata specificità dei pannelli multigenici utilizzati per la diagnosi di epilessia ad esordio precoce. La frequenza delle comorbidità riportate e l’impatto di queste sul fenotipo dei soggetti esaminati rafforzano l’importanza di considerare l’inclusione dell’epilessia dell’età evolutiva nella famiglia delle NDDs. Una diagnosi molecolare precoce e accurata permette di pianificare un percorso assistenziale multidisciplinare personalizzato e di avere accesso a trattamenti terapeutici mirati e ad una consulenza genetica adeguata.
