Canine splenic hemangiosarcoma is an extremely aggressive tumor with a poor prognosis related to the high metastatic capacity. The heterogeneity and aggressiveness that characterize the hemangiosarcoma represent a diagnostic and therapeutic challenge for the clinician and pathologist, reason why this tumor has been the subject of numerous studies. In this study, the morphological histological characteristics of the neoplasm and the expression of four immunohistochemical markers were analyzed in order to further understand the biological behavior of the tumor. The morphological parameters and the results of the marker’s immunohistochemical expression were analyzed to research correlations with clinical data of the subjects from which the study samples were taken. In addition, a measure of the level of agreement among the pathologists responsible for the morphological assessment was conducted. No significant correlations emerged from this study between the investigated parameters and the clinical outcome of the subjects, except for the correlation detected between mitotic count and clinical stage; however, this finding does not appear to have prognostic significance. Finally, the low level of agreement reached among pathologists confirms the high morphological heterogeneity of hemangiosarcoma which inevitably leads to variable and subjective evaluations among observers, even among experienced pathologists.
L’emangiosarcoma splenico canino è una neoplasia estremamente aggressiva con una prognosi infausta dovuta all’elevata capacità metastatica. L’eterogeneità e l’aggressività che caratterizzano l’emangiosarcoma rappresentano una sfida diagnostica e terapeutica per il clinico e il patologo, motivo per cui questo tumore è stato oggetto di numerosi studi. In questo studio sono state analizzate le caratteristiche morfologiche istologiche della neoplasia e l’espressione di quattro marcatori immunoistochimici al fine di comprendere ulteriormente il comportamento biologico del tumore. I parametri morfologici e i risultati dell’espressione immunoistochimica dei marcatori sono stati analizzati per ricercare delle correlazioni con i dati clinici dei soggetti dai quali sono stati prelevati i campioni dello studio. Inoltre, è stata condotta una misurazione del livello di concordanza tra i patologi responsabili delle valutazioni morfologiche. Da questo studio non sono emerse correlazioni significative tra i parametri indagati e l’outcome clinico dei soggetti, ad eccezione della correlazione rilevata tra la conta mitotica e lo stadio clinico; tuttavia, tale risultato non sembra avere significato prognostico. Infine, lo scarso livello di concordanza raggiunto tra i patologi conferma la l’elevata eterogeneità morfologica dell’emangiosarcoma che porta, inevitabilmente, a valutazioni variabili e soggettive tra gli osservatori, anche tra patologi esperti.
