Innovation and research are very important themes in Azienda Ospedale Università Padova, which has the obligation to be updated with the evolution of technologies and working methods in order to guarantee to every patient the best sanitary assistance. The following paper illustrates the principal methods of approach used in procedures of evaluation of new electromedical devices and electronic instruments intended for sanitary use that are offered to the company. In these procedures medical opinions are extremely important to get the right evaluation under a clinical aspect; on the other hand, it is impossible to neglect the economical contribution when evaluating the purchase of an innovative technology for the hospital wards. An electrosurgical device in recent dotation in the ward of plastic surgery is going to get taken into consideration: it is an electrosurgical unit based on a system of generation of electric current using the ionization at high energy of Helium, delivered in the form of plasma. The theme of infungibility is going to get examinated, introduced by the use of technologies like the one previously mentioned, that brings a relevant problem in terms of costs and knowledges. Later, the most important aspects in matter of electrosurgical units will be evaluated, like the workings, clinical applications and ease of use, speaking of risks and hazards that can occur while working with this type of technology. The plasma device will be compared with similar instruments available on the market, in order to underline the innovations and improvements that the new technology brings. Finally, some studies on the working of the electrosurgical unit on animal tissues will be presented, talking in a particular way of the usage of the electrosurgical device.
L’innovazione e la ricerca sono temi cardine nell’Azienda Ospedale Università di Padova, che ha il dovere di essere sempre al passo con l’aggiornamento delle tecnologie e dei metodi di lavoro per garantire ai pazienti la migliore assistenza sanitaria possibile. Il presente elaborato illustra i principali metodi di approccio utilizzati nelle procedure di valutazione di nuovi apparecchi elettromedicali e strumenti elettronici ad uso sanitario che vengono offerti all’Azienda. In queste procedure è di fondamentale importanza il parere dei medici per una adeguata valutazione sul piano clinico; d’altra parte, non è trascurabile l’aspetto economico quando si valuti l’acquisto di una tecnologia innovativa da proporre nei reparti ospedalieri. Verrà preso in esame in modo particolare un apparecchio per elettrochirurgia in recente dotazione al reparto di chirurgia plastica: si tratta di un elettrobisturi che è basato su di un sistema di generazione di corrente innovativo che sfrutta la ionizzazione ad alta energia del gas elio, erogato sotto forma di plasma. Si analizzerà il tema dell’infungibilità, introdotto da tecnologie come quella in esame, che si presenta all’Azienda come un problema non indifferente in termini di costi e conoscenze. Saranno poi trattati gli aspetti più importanti in materia di apparecchiature per elettrochirurgia quali funzionamento, applicazione clinica, modalità di utilizzo, tenendo in considerazione anche i rischi che ne possono derivare per la salute e la sicurezza del paziente. L’elettrobisturi al plasma sarà confrontato con strumenti analoghi commercialmente disponibili, per evidenziare i miglioramenti e le innovazioni introdotti dalla nuova tecnologia. Verranno infine esposti alcuni studi riguardanti l’effetto dell’elettrobisturi su tessuti animali, descrivendo nello specifico l’impiego e la modalità di utilizzo dell’elettrobisturi.
