Currently, lung cancer represents the leading cause of death from malignancy worldwide. In the past decade, important changes in the molecular characterization of lung cancer have been observed. In particular, the identification of oncogenic driver mutations and the concept of oncogene addiction have changed the therapeutic approach for patients with advanced lung cancer. In addition, the recent introduction of immunotherapy has further revolutionized the therapeutic approach; such therapy is initiated upon evaluation of the expression level of the PD-L1 marker, which, however, has not proven to be a robust predictive marker of therapeutic response. Tissue biopsy represents the gold standard approach for molecular investigations; however, it is not always feasible, and the material collected may not be suitable for said analysis. Circulating free DNA (cfDNA) analysis, also known as liquid biopsy, represents an alternative and complementary approach to investigate the mutational status of the tumour. It is minimally invasive, easily repeatable and willingly accepted by the patient. The aim of this thesis is to illustrate how this method, combined with the next-generation sequencing approach, can be a useful tool for monitoring therapeutic effects in patients with lung cancer treated with immunotherapy.

Il tumore del polmone rappresenta attualmente la principale causa di morte per neoplasia a livello mondiale. Nell'ultimo decennio si sono osservati importanti cambiamenti nel campo della caratterizzazione molecolare del tumore del polmone. In particolare, l’identificazione di mutazioni in oncogeni driver e il concetto di dipendenza della crescita tumorale da tali oncogeni ha modificato l'approccio terapeutico per i pazienti con tumore del polmone avanzato. Inoltre, l’introduzione dell’immunoterapia ha recentemente e ulteriormente rivoluzionato l’approccio terapeutico; viene avviata previa valutazione del livello di espressione del marcatore PD-L1 che, tuttavia, non si è dimostrato un marker predittivo robusto di risposta terapeutica. La biopsia tissutale rappresenta l’approccio di elezione per le indagini molecolari; tuttavia, non sempre è eseguibile ed il materiale prelevato può risultare non idoneo per le analisi molecolari. L'analisi del DNA libero circolante (cfDNA), nota anche come biopsia liquida, rappresenta uno strumento alternativo e complementare per definire lo stato mutazionale del tumore. Si tratta di un approccio poco invasivo, accettato dal paziente e facilmente ripetibile. Scopo della tesi è quello di illustrare come tale metodica, se unita ad un approccio di sequenziamento di nuova generazione, possa rappresentare uno strumento utile per monitorare gli effetti terapeutici in pazienti con tumore del polmone trattati con immunoterapia.
