The pharmaceutical industry is highly focused on the development of new biologic drugs, especially monoclonal antibodies, which have a considerable number of benefits over the traditional drugs. High efficacy, specificity and tolerance are just some of the important positive aspects that these new types of drugs are able to provide. However, they are intrinsically unstable. In fact, these drugs are very high molecular weight proteins that can undergo physical and chemical degradation phenomena such as oxidation, denaturation, and aggregation. These modifications can make the drug less effective or even affect its safety profile. Therefore, the pharmaceutical industry, in addition to focusing on the development of the active ingredient itself, uses considerable resources to identify a formulation that will best stabilize antibodies. After identifying the optimal formulation, industries perform a series of tests required by regulatory agencies, which for many countries refer to the ICH QC5 guidelines, specific to biotechnological products. The data required to obtain marketing authorization for the new drug, however, are often focused on preclinical rather than post-production level analyses. In this thesis project, we decided to focus on studying the photostability in artificial sunlight (outdoor and indoor) of Cemiplimab in the Libtayo® formulation, comparing stability data even in the absence of the excipients present in the original drug. The formulation and the purified active principle were also subjected to specific radiation in the UVB (312 nm) and UVA (365 nm) to investigate which radiation was really responsible for the light-induced changes and whether there were differences between those induced at 312 nm and those at 365 nm. A set of chemico-physical analyses (UV-Vis in second derivative, Fluorescence, Circular Dichroism, SEC-UHPLC, SDS-PAGE) were carried out by diluting the isolated antibody and Libtayo® formulation to the concentration of 1 mg/mL with sterile glucose solution (5%) and saline solution (0.9%), i.e., in the range of concentrations used for patient administration. Dilution and all the subsequent steps to which the drug is subjected (transport to the site of administration and exposure to light) may in fact induce changes that could affect its efficacy and safety as well. It has been found that in most cases excipients play a key role in protecting the active principle from the damaging effects of light (oxidation and aggregation), especially in glucose solution where glucose degradation products, formed during the sterilization process, increase susceptibility to irradiation. However, especially due to the strong dilution, excipients have not always been able to ensure its stability.

Il mercato farmaceutico è altamente concentrato nello sviluppo di nuovi farmaci biologici, specialmente anticorpi monoclonali, i quali presentano una notevole serie di benefici rispetto ai farmaci tradizionali. Alta efficacia, specificità e tolleranza sono solo alcuni degli importanti aspetti positivi che questi nuovi tipi di farmaci sono in grado di assicurare. Tuttavia, essi sono intrinsecamente instabili. Questi farmaci, difatti, sono proteine ad altissimo peso molecolare che possono essere sottoposte a fenomeni di degradazione fisica e chimica come ossidazioni, denaturazioni ed aggregazioni. Queste modifiche possono rendere il farmaco meno efficace o addirittura ledere al profilo di sicurezza di quest’ultimo. Perciò, l’industria farmaceutica, oltre a concentrarsi sullo sviluppo del principio attivo stesso, utilizza notevoli risorse per individuare una formulazione che permetta di stabilizzare al meglio gli anticorpi. Dopo aver individuato la formulazione ottimale, le industrie eseguono una serie di test richiesti dagli enti regolatori che per molti paesi fanno riferimento alle linee guida ICH QC5, specifiche per i prodotti biotecnologici. I dati richiesti per ottenere l’autorizzazione all’immissione in commercio del nuovo farmaco, però, sono spesso incentrati sulle analisi a livello preclinico rispetto a quelle di post-produzione. In questo progetto di tesi abbiamo deciso di concentrarci sullo studio della fotostabilità alla luce solare artificiale (indoor e outdoor) di Cemiplimab nella formulazione Libtayo®, confrontando i dati di stabilità anche in assenza degli eccipienti presenti nel farmaco originale. La formulazione ed il principio attivo purificato sono stati sottoposti anche a specifiche radiazioni nell’UVB (312 nm) e UVA (365 nm) per conoscere quali radiazioni fossero realmente responsabili delle modifiche indotte dalla luce e se ci fossero differenze tra quelle indotte a 312 nm e quelle a 365 nm. Un set di analisi chimico-fisiche (UV-Vis in derivata seconda, Fluorescenza, Dicroismo Circolare, SEC-UHPLC, SDS-PAGE) sono state effettuate diluendo l’anticorpo isolato e la formulazione Libtayo® alla concentrazione di 1 mg/mL con soluzione glucosata sterile (5%) e soluzione salina (0.9%), ovvero nel range di concentrazioni impiegate per la somministrazione al paziente. Anche la diluizione e tutte le fasi successive a cui viene sottoposto il farmaco (trasporto alla sede di somministrazione ed esposizione alla luce), possono infatti indurre modifiche che potrebbero alterarne l’efficacia e la sicurezza È stato riscontrato che nella maggior parte dei casi gli eccipienti giocano un ruolo fondamentale nel proteggere il principio attivo dall’effetto dannoso della luce (ossidazione e aggregazione), soprattutto in soluzione glucosata dove i prodotti di degradazione del glucosio formatisi durante la sterilizzazione aumentano la suscettibilità all’irradiazione. Tuttavia, specialmente grazie alla forte diluzione, gli eccipienti non sono sempre stati in grado di assicurarne la stabilità.
