Introduction: The therapeutic relationship is a fundamental tool for the nurse who works in the field of Mental Health. A correct approach can favor the alliance necessary to reach an adequate therapeutic adherence but some factors can constitute obstacles that do not allow the establishment of the optimal relationship. Strategies must therefore be put in place to improve the relationship of trust that is desirable between the nurse and the patient. Aim: investigate the role of the nurse in the therapeutic alliance with the schizophrenic patient going to analyze the factors that hinder and strengthen it. Materials and Methods: The articles were searched on pubmed using search strings composed of free words, mesh terms and boolean operators. They were then selected in a first phase after reading the title and abstract. Those deemed relevant to the purpose of the research were analyzed completely by reading the entire text (full text). Literature synthesis: factors that may compromise the relationship between nurse and schizophrenic patient are related to organizational, bureaucratic and participatory factors, makes the patient feel excluded, isolated and has negative consequences in adherence to therapy as well as in the relationship with the health care professional. To improve it it is necessary to know how to involve them in the decisions regarding their treatments, taking into account their preferences and habits. This improves and strengthens the relationship between the two figures and increases adherence to treatments. Discussion: Schizophrenic patients complain that nurses spend little time with them because of the workload, the increase in bureaucracy, the reduction in staff, and feel excluded from decisions regarding their care path. It is therefore important that the nurse knows how to involve the patient, especially to establish that alliance that is the basis of optimal therapeutic adhesion. Conclusion: it is essential for the nurse to involve the patient affected by schizophrenia in the decisionsregarding histreatments through a "Shared decision-making" approach ("SDM"): this, through the strengthening of the therapeutic relationship, may result in a consolidation of that patient-treating alliance necessary for optimal adhesion to treatments. Introduzione: la relazione è uno strumento fondamentale per l’infermiere che opera nell’ambito della Salute Mentale. Un giusto approccio può favorire quell’alleanza necessaria per raggiungere un’adeguata aderenza terapeutica ma alcuni fattori possono costituire ostacoli che non permettono l’instaurarsi della relazione ottimale. Bisogna, dunque, attuare delle strategie che permettano di perfezionare quel rapporto di fiducia auspicabile tra infermiere e paziente. Scopo: indagare il ruolo dell’infermiere nell’alleanza terapeutica con il paziente schizofrenico andando ad analizzare i fattori che la ostacolano e che la rafforzano. Materiali e metodi: gli articoli sono stati ricercati su PubMed utilizzando stringhe di ricerca composte da parole libere, termini Mesh e operatori booleani. Sono stati poi selezionati in una prima fase previa lettura di titolo ed abstract. Quelliritenutipertinenti allo scopo della ricerca sono stati analizzati completamente tramite lettura dell’intero testo (full text). Sintesi della letteratura: i fattori che possono compromettere la relazione tra infermiere e paziente schizofrenico sono legati a fattori organizzativi, burocratici e partecipativi, fa sentire il paziente escluso, isolato ed ha delle ripercussioni negative nell’aderenza alla terapia oltre che nella relazione con l’operatore sanitario. Per migliorarla è necessario saper coinvolgerli soprattutto nelle decisioni che riguardano i loro trattamenti tenendo conto delle loro preferenze e abitudini. Questo migliora e rafforza la relazione tra le due figure e aumenta l’aderenza ai trattamenti. Discussione: i pazienti schizofrenici recriminano il fatto che gli infermieri dedicano loro poco tempo a causa del carico di lavoro, dell’aumento della burocrazia, della riduzione del personale e si sentono esclusi dalle decisioni riguardanti il loro percorso di cure. È dunque importante che l’infermiere sappia coinvolgere il paziente, soprattutto per instaurare quell’alleanza che è alla base dell’ottimale aderenza terapeutica. Conclusioni: è fondamentale che l’infermiere renda partecipe il paziente affetto da schizofrenia delle decisioni riguardanti i suoi trattamenti tramite un approccio “Shared decision-making” (“SDM”): questo, attraverso il rafforzamento della relazione terapeutica, potrà tradursi in un consolidamento di quell’alleanza paziente-curante necessaria per una ottimale adesione ai trattamenti.
