In an attempt to better understand the impact of the Covid-19 pandemic on the mental health of children, a follow-up study was carried out between 12-18 months after hospitalization of the neuropsychiatric patient. The aim of the study is to investigate the effects of the pandemic on the psychophysical well-being and social functionality of neuropsychiatric patients 12-18 months after hospitalization. An improvement in patients' problems at follow-up after hospitalization in the pandemic era has been hypothesized, by virtue of a relaxation of the restrictive measures and a continuation of treatment therapies. It has been hypothesized that pandemic variables, such as any positivity to Sars-Cov-2, quarantines and distance learning, which occurred in this period of time, may have influenced the mental health of neuropsychiatric patients. A longitudinal retrospective cohort observational study was carried out, in which patients admitted to the Child Neuropsychiatry facility of reference for North-East Italy were examined. The selected patients are children or adolescents aged 0-17 hospitalized between September 2020 and November 2021. The clinical tool used to investigate the impact on psychophysical well-being and social functioning is the HoNOSCA scale, observing the scores three different times: at the beginning t0 and at the end of the hospitalization t1, and at the follow-up t2. In the first part of the work, the socio-demographic and clinical anamnestic data of 124 patients were collected through an accurate review of the hospital material. In the second part, 124 semi-structured telephone interviews were carried out with the patients' parents, excluding 28 participants, due to parents' refusal to release the information, or due to unavailability by telephone. During these interviews, data related to post-hospitalization care, data related to Covid-19, and clinical data related to the last two weeks were retrieved using the HoNOSCA tool. A total of 96 patients were enrolled, of which 78 females and 18 males with a mean age of 15.6. Patients were hospitalized between September 2020 and November 2021, of which 19 in 2020 and 77 in 2021, and most of these received a diagnosis of internalizing disorder at discharge. Using the HoNOSCA scale, there was a reduction in the mean of the total scores between t0 and t1, and this with t2. Comparing t0-t1, a significant reduction of all HoNOSCA items between t0 and t1 emerges. There are no significant differences for sociodemographic and family variables. For the clinical variables at admission, the mean scores are significantly higher in patients hospitalized in 2020 than in those hospitalized in 2021, and there is a significant difference for internalizing, externalizing and eating disorders. Regarding the comparison at t2, almost all of the HoNOSCA items between t1 and t2 were significant. No significant differences emerged for the clinical variables at admission, for those following admission and for the pandemic variables, except in this last for the quarantine period, where higher scores appear for those who experienced the quarantine in 2020, compared to those who experienced it in 2021 or 2022. In conclusion, the hypothesis of an improvement in the mental health conditions of the study sample, by virtue of a reduced pandemic pressure of the last period and of a fundamental role of the continuation of treatments over time. The hypothesis of the influence of pandemic variables on mental health, such as the positivity to Sars-Cov-2, the quarantine experienced or the DAD, has not been confirmed, leading to think that the impact on the mental health of the patients was probably influenced by the general pandemic context faced in recent years.

Nel tentativo di comprendere al meglio l'impatto della pandemia da Covid-19 sulla salute mentale dei ragazzi, è stato realizzato uno studio di follow-up tra i 12-18 mesi dal ricovero del paziente neuropsichiatrico. L’obiettivo dello studio è quello di indagare gli effetti della pandemia sul benessere psicofisico e funzionalità sociale dei pazienti neuropsichiatrici a 12-18 mesi dal ricovero. È stato ipotizzato un miglioramento delle problematiche dei pazienti al follow-up dal ricovero in epoca pandemica, in forza di un allentamento delle misure restrittive e di un proseguimento delle cure. È stato ipotizzato che variabili pandemiche, come la positività al Sars-Cov-2, le quarantene e la DAD, presentatisi in questo arco di tempo, possano aver influito sulla salute mentale dei pazienti neuropsichiatrici. È stato svolto uno studio osservazionale longitudinale di coorte retrospettivo, in cui sono stati presi in esame i pazienti ricoverati presso la struttura di Neuropsichiatria Infantile di riferimento per il Nord Est Italiano. I pazienti selezionati sono bambini o adolescenti 0-17 anni ricoverati nel periodo di tempo compreso tra settembre 2020 e novembre 2021. Lo strumento clinico utilizzato per indagare l’impatto sul benessere psicofisico e sulla funzionalità sociale è la scala HoNOSCA, osservandone i punteggi rilevati in tre diversi tempi: all’inizio t0 e alla fine del ricovero t1, e al follow-up t2. Nella prima parte del lavoro sono stati raccolti i dati sociodemografici e clinico-anamnestici di 124 pazienti tramite accurata revisione del materiale ospedaliero. Nella seconda parte, sono state realizzate 124 interviste telefoniche semi strutturate ai genitori dei pazienti, escludendo 28 partecipanti, per rifiuto da parte dei genitori a rilasciare le informazioni, o per mancata reperibilità telefonica. Durante queste interviste, sono stati recuperati dati relativi alle cure successive al ricovero, dati relativi al Covid-19, e dati clinici relativi alle ultime due settimane, tramite lo strumento HoNOSCA. Complessivamente sono stati arruolati 96 pazienti, di cui 78 femmine e 18 maschi con età media di 15.6. I pazienti sono stati ricoverati tra settembre 2020 e novembre 2021, di cui 19 nel 2020 e 77 nel 2021, e la maggior parte di questi ha ricevuto una diagnosi di disturbo internalizzante alla dimissione. Utilizzando la scala HoNOSCA, è risultata una riduzione delle medie dei punteggi totali tra t0 e t1, e questo con t2. Al confronto t0-t1, emerge una riduzione significativa di tutti gli item di HoNOSCA tra t0 e t1. Non ci sono differenze significative per le variabili sociodemografiche e famigliari. Per le variabili cliniche al ricovero, le medie dei punteggi sono significativamente più elevate nei pazienti ricoverati nel 2020, rispetto a coloro ricoverati nel 2021, ed è presente una differenza significativa per i disturbi internalizzanti, esternalizzanti e alimentari. Circa il confronto a t2, sono risultati significativi quasi tutti quasi tutti gli item di HoNOSCA tra t1 e t2, Non sono emerse differenze significative per le variabili cliniche al ricovero, per quelle successive al ricovero e per le variabili pandemiche, ad eccezione in quest’ultima per il periodo di quarantena, dove dove appaiono punteggi maggiori per chi ha vissuta la quarantena nel 2020, rispetto a chi l’ha vissuta nel 2021 o 2022. In conclusione, è stata confermata l’ipotesi del miglioramento delle condizioni di salute mentale del campione in studio, in virtù di una ridotta pressione pandemica dell’ultimo periodo e di un fondamentale ruolo del proseguimento delle cure nel tempo. Non è stata confermata l’ipotesi di influenza delle variabili pandemiche sulla salute mentale, come la positività al Sars-Cov-2, la quarantena vissuta o la DAD, portando a pensare che l’impatto sulla salute mentale dei ragazzi sia probabilmente stato influenzato dal generale contesto pandemico affrontato negli ultimi anni.

DISTURBI NEUROPSICHIATRICI DELL'ETÀ EVOLUTIVA AL TEMPO DEL COVID-19: STUDIO CLINICO E DI FOLLOW-UP DI PAZIENTI RICOVERATI

Abstract
