The study described in this thesis project aims to evaluate the impact of a guided self-help intervention for patients with Bulimia Nervosa or Binge Eating Disorder. The theoretical background underlying this treatment refers on the one hand to Fairburn's cognitive-behavioral model of self-help, on the other hand to the cognitive-interpersonal model of Treasure and Schmidt. The focus is on psychological distress (symptoms of depression, anxiety and stress), assessed through the Depression Anxiety Stress Scales (DASS-21), and on the perception of quality of life (assessed through the Eating Disorder Quality of Life, IEDQOL). Study participants were recruited through the Regional Centers for Eating Disorders of Padua and Chieti, and were followed by therapeutic guides. The latter were adequately trained in motivational interviewing and guided self-help intervention, and were supervised on a weekly basis. The material provided to patients consists of a manual focused on the phenomenon of binge eating, and some video recordings of testimonies from former patients or expert advice. The study revealed a statistically significant improvement in the psychological, physical/cognitive and interpersonal frameworks of the patients' quality of life. A trend of reduction in the symptoms of depression, anxiety and stress was also found, despite not being statistically significant.
Lo studio descritto nel presente progetto di tesi ha l’obiettivo di valutare l'impatto di un intervento di auto-aiuto guidato per pazienti con Bulimia Nervosa o Disturbo da Alimentazione Incontrollata. Il background teorico alla base di questo trattamento fa riferimento da una parte al modello cognitivo-comportamentale di auto-aiuto di Fairburn, dall’altra al modello cognitivo-interpersonale di Treasure e Schmidt. Il focus è sul distress psicologico (sintomi di depressione, ansia e stress), valutato attraverso la Depression Anxiety Stress Scales (DASS-21), e sulla percezione della qualità di vita (stimata mediante l’Eating Disorder Quality of Life, IEDQOL). I partecipanti allo studio sono stati reclutati attraverso i Centri Regionali per i Disturbi Alimentari di Padova e Chieti, e sono stati seguiti da guide terapeutiche. Queste ultime sono state adeguatamente formate sul colloquio motivazionale e sull’intervento di auto-aiuto guidato, e sono state supervisionate su base settimanale. Il materiale fornito ai pazienti consiste in un manuale incentrato sul fenomeno delle abbuffate, ed in alcune videoregistrazioni di testimonianze di ex pazienti o di consigli di esperti. Dallo studio è emerso un miglioramento statisticamente significativo nei quadri psicologico, fisico/cognitivo ed interpersonale della qualità di vita dei pazienti. Si è poi riscontrata una tendenza alla riduzione dei sintomi di depressione, ansia e stress, seppur non statisticamente significativa.
