La formazione di base del Fisioterapista in Cure Palliative: l'esperienza di tirocinio nel Corso di Laurea in Fisioterapia dell'Università degli Studi di Padova.

Abstract Introduction: This thesis project arises from the growing interest in themes like end-of-life care and holistic approaches for people with advanced life-limiting and life-threatening illnesses. One of the main problems is the necessity of palliative care and specific treatment for people with chronic and degenerative pathologies. This social and health need is closely related to the need for adequate education for health professionals involved in palliative and end-of-life care. Purpose of the study: This study aims to define how the pilot project of an internship in palliative care has been arranged and presented to the students of the Physiotherapy Degree Course of the University of Padua. The research has two primary purposes: first, to define the skills and abilities that the students must acknowledge and the main learning goals of the internship; second, to explore how the students involved and their internship guides have lived this experience and what they think about it. Materials and Methods: Different methodological approaches have helped delve into the topic of the basic training of physiotherapists in palliative care: first, a review of the existing literature and the current legislation about palliative care; secondly, a detailed report of all the actions that took place and finally the analysis of the data collected through semi-structured interviews and scripts. Background: This research project starts with the World Health Organization's definition of palliative care and continues with a summary of Italian laws regarding access to palliative and end-of-life care and the undergraduate education of health professionals. Results: Most Italian universities tried to update their curricular offer, trying to implement L.38/2010 about palliative care and the education of health professionals. During the academic year 2022/2023, the students of the Physiotherapy Degree Course of the University of Padua were allowed to attend training in specialist palliative care services. All the students and all the training guides appreciated the experience and the opportunity given to them. Discussion: The introduction of this training answers the need to update undergraduate education to emerging social and health needs, due to epidemiological and demographic changes in our society. The analysis of this experience highlights what aspects are the most formative and what could be improved or changed in the future. Conclusion: The University must guarantee adequate education to the students, giving them all the knowledge and the necessary tools to carry out the profession of physiotherapist in different settings. It must prepare the students to work with chronic pain and degenerative illnesses and deal with advanced stages of pathology and end-of-life conditions.

Riassunto Introduzione: Questo progetto di tesi nasce dall’interesse ai temi dell’accompagnamento nel fine vita e della presa in carico di persone in fase avanzata e terminale di malattia. Tra i bisogni di salute emergenti vi è, infatti, il crescente bisogno di cure palliative e di un’adeguata gestione della cronicità e di casi ad alta complessità. Tale bisogno sociale e di salute è strettamente correlato al bisogno di formazione del personale sanitario che si occupa di cure palliative e di terapia del dolore. Obiettivo dello Studio: Il presente elaborato mira a definire le modalità con cui è stato proposto in via sperimentale il tirocinio nell’ambito delle cure palliative nel Corso di Laurea in Fisioterapia dell’Università degli Studi di Padova. Si pone due obiettivi fondamentali, innanzitutto definire le conoscenze e le competenze da sviluppare e gli obiettivi formativi identificati, in seguito analizzare il vissuto da parte degli studenti che hanno scelto di aderire e delle guide di tirocinio coinvolte. Materiali e Metodi: Il tema della formazione di base del Fisioterapista in cure palliative è stato approfondito grazie a diversi approcci metodologici: un’iniziale ricerca bibliografica della normativa italiana in materia di cure palliative e ricerca nelle principali banche dati scientifiche, la registrazione puntuale delle attività svolte per l’organizzazione e l’attuazione del tirocinio, l’analisi dei dati ottenuti tramite interviste semi-strutturate e narrazioni scritte, secondo i principi della ricerca qualitativa. Quadro teorico: Il lavoro di ricerca muove i suoi primi passi dalla definizione di cure palliative dell’Organizzazione Mondiale della Sanità e continua, poi, con un breve excursus delle principali normative italiane in materia di accesso alle cure palliative e di formazione del personale sanitario. Risultati: Gli Atenei italiani hanno progressivamente aggiornato la propria offerta curriculare per recepire le indicazioni della Legge 38/2010 con proposte di formazione rispetto alle cure palliative. Nell’anno accademico 2022/2023 agli studenti del Corso di Laurea in Fisioterapia dell’Università degli Studi di Padova è stata offerta la possibilità di svolgere un tirocinio in servizi che erogano cure palliative specialistiche. Nel complesso, l’esperienza è stata accolta positivamente da tutte le persone coinvolte ed è stata apprezzata l’opportunità di mettersi alla prova in situazioni complesse. Discussione: L’introduzione di questo tirocinio sperimentale risponde all’esigenza di adattare la proposta formativa universitaria ai bisogni socio-sanitari emergenti e ai cambiamenti epidemiologici e demografici in atto nella nostra società. È di grande interesse analizzare come sia stata accolta e vissuta quest’esperienza, quali aspetti siano stati ritenuti più utili e formativi e quali invece si potrebbero migliorare, nell’ottica di riproporre nuovamente il tirocinio in futuro. Conclusioni: L’Università deve garantire una formazione completa agli studenti, fornendo loro gli strumenti indispensabili per svolgere la professione di Fisioterapista in diversi contesti assistenziali e di cura, preparandoli anche nel campo della cronicità e della fase avanzata e terminale di malattia.
