INTRODUCTION: Introduction: the Check-list is a tool mainly used in the operating plate with the purpose of preventing errors and reducing the risk of occurrence of complications in the intra- and post surgery. It consists of 3 steps: sign In,; Time Out, and Sign Out. The responsibility for completing this tool lies with the nurse and being standardized needs immediate understanding and easy compilation, both in elective and emergency/urgency cases. MATERIALS AND METHODS: The present study examined the compilation and analyzed the use of the Checklist in the operating room by the nursing staff at the operating room of Bassano Hospital. The following were taken into analysis. ten checklists applied in five different surgical branches (Gynecology, General Surgery, Orthopedics, Vascular Surgery and Urology). The study also also included a short interview with ten operating room nurses with the aim of identify difficulties encountered in completing and reading the checklist. Finally, a literature review was performed to detect possible improvements in the use of the checklist in the operating room. RESULTS: It appears from the study conducted that the compilation of the checklist does not occurs within the timeframe established by the World Health Organization and often its reading is not performed. This results, therefore, in poor communication between the figures physicians and nurses, responsible for operating room activities. The interviews with nurses also highlighted the lack of specificity of certain checklist items such as those dedicated to the estimation of blood loss and the difficult understanding of others related to postoperative management, causing them to be therefore an incorrect compilation. CONCLUSIONS: The study confirms that appropriate use of the Checklist is essential to ensure improved patient safety in the operating room and proper conduct of operating room activities. It probably needs a greater awareness of its use by operating room staff in order to ensure the maximum effectiveness of a tool that was created precisely to reduce the risk of error in the operating room.
INTRODUZIONE: la Check-list è uno strumento utilizzato prevalentemente nella piastra operatoria con lo scopo di prevenire errori e ridurre il rischio di insorgenza di complicanze nell’intra e post operatorio. Essa si compone di 3 fasi: sign In, Time Out, e Sign Out. La responsabilità di compilazione di questo strumento è dell’infermiere ed essendo standardizzata necessita di immediata comprensione e di facile compilazione, sia nei casi di elezione che nei casi di emergenza/urgenza MATERIALI E METODI: Il presente studio ha esaminato la compilazione e analizzato l'utilizzo della Checklist in sala operatoria da parte del personale infermieristico presso la piastra operatoria dell’Ospedale di Bassano. Sono state prese in analisi dieci checklist applicate in cinque diverse branche chirurgiche (Ginecologia, Chirurgia Generale, Ortopedia, Chirurgia Vascolare e Urologia). Lo studio ha incluso anche una breve intervista a dieci infermieri di sala operatoria con lo scopo di identificare le difficoltà riscontrate nella compilazione e nella lettura della checklist. Infine è stata eseguita una revisione della letteratura per rilevare possibili miglioramenti nell’utilizzo della checklista in sala operatoria. RISULTATI: Dallo studio condotto emerge che la compilazione della Checklist non avviene nei tempi stabiliti dall’Organizzazione Mondiale della Sanità e spesso la sua lettura non viene eseguita. Ne deriva, quindi una scarsa comunicazione tra le figure mediche ed infermieristiche, responsabili delle attività di sala operatoria. Le interviste con gli infermieri hanno evidenziato, inoltre, la poca specificità di alcuni item della checklist come quelli dedicati alla stima delle perdite ematiche e la difficile comprensione di altri relativi alla gestione postoperatoria, che ne causano quindi una scorretta compilazione. CONCLUSIONI: Lo studio conferma che un adeguato utilizzo adeguato della Checklist è fondamentale per garantire una maggiore sicurezza del paziente in sala operatoria e un corretto svolgimento delle attività di sala operatoria. Probabilmente serve una maggiore sensibilizzazione al suo utilizzo da parte del personale di sala operatoria per poter garantire la massima efficacia di uno strumento nato proprio per ridurre il rischio di errore in sala operatoria.
