INTRODUCTION: Parkinson's disease, is a degenerative disorder consisting of a series of motor symptoms such as tremor at rest, bradykinesia, gait alteration and rest and non-rest tremor, which tend to worsen over time leading to a deflection of the person's health. These affect the performance of both ADLs (activity daily living) and IADLs (instrumental activity daily living), leading to a decrease in quality of life as well as an increase in caregiver burden. Rehabilitation therapy in addition to drug therapy has been shown to bring additional benefits to the person with Parkinson's disease and his or her family. In a multidisciplinary perspective, the figure of the Occupational Therapist becomes crucial in order to maintain the highest possible level of autonomy. OBJECTIVE: The aim of this paper is to carry out a literature review with the intention of going to investigate the effectiveness of the Occupational Therapy intervention, making an overview of the interventions currently existing in the rehabilitation field. MATERIALS and METHODS: To answer the thesis question, a literature search was conducted where articles from 2013 to 2023 were selected. The following databases were used: Pubmed, American Journal of Occupational Therapy, Cochrane. The keywords used were: "Parkinson's Disease", "Parkinson's", "Occupational Therapy", "Occupational Therapy Intervention", "rehabilitation", "home training", "caregivers", "quality of life", "guide lines", "home", "environment", "community". RESULTS AND CONCLUSIONS: The studies show that the TO can perform different types of treatments in PD in relation to the person's health status and needs, while also bringing benefits to the caregiver. The data show that the Occupational Therapy intervention leads to improvements in self-perception of performance, quality of life, upper limb function, mental health status, and reduce the caregiver's care burden. However, further studies are needed to show what the added value of TO is in PD, its effectiveness in different stages of the disease, and how it can contribute to relieving caregiver care burden.
INTRODUZIONE: La Malattia di Parkinson (MdP) è una patologia degenerativa che si manifesta con una serie di sintomi motori come tremore a riposo, bradicinesia, alterazione dell’andatura e non, che con il tempo tendono a peggiorare determinando una deflessione della salute della persona. Questi sintomi vanno ad impattare nello svolgimento sia delle ADL (activity of daily living) che delle IADL (instrumental activity of daily living), portando ad una diminuzione della qualità di vita nonché all’aumento del carico assistenziale del caregiver. La terapia riabilitativa, in aggiunta alla terapia farmacologica ha dimostrato di portare ulteriori benefici alla persona con Malattia di Parkinson e alla sua famiglia. In un’ottica multidisciplinare, la figura del Terapista Occupazionale (TO) diventa fondamentale per mantenere il più alto livello di autonomia possibile. OBIETTIVO: L’obiettivo di questo elaborato è quello di svolgere una revisione in letteratura con l’intento di andare ad indagare l’efficacia dell’intervento di Terapia Occupazionale, aggiungendo una panoramica degli interventi attualmente esistenti in ambito riabilitativo. MATERIALI e METODI: Per rispondere al quesito di tesi è stata condotta una ricerca in letteratura dove sono stati selezionati articoli dal 2013 al 2023. Sono stati usati i seguenti database: Pubmed, American Journal of Occupational Therapy, Cochrane. Le parole chiave utilizzate sono state:“Parkinson’s Disease”, “Parkinson”, “Occupational Therapy”, “Occupational Therapy Intervention”, “rehabilitation”, “home training”, “caregivers”, “quality of life”, “guide lines”, “home”, “environment”, “community”. RISULTATI E CONCLUSIONI: Dagli studi emerge come il TO possa eseguire diversi tipi di trattamenti nella MdP in relazione allo stato di salute della persona e alle sue esigenze, portando anche dei benefici al caregiver. I dati dimostrano che l’intervento di Terapia Occupazionale porta a dei miglioramenti a livello dell’autopercezione della performance, della qualità di vita, della funzione dell’arto superiore, dello stato di salute mentale e riducano il carico assistenziale del caregiver. Tuttavia si rendono necessari ulteriori studi che dimostrino quale sia il valore aggiunto della TO nella MdP, la sua efficacia nei diversi stadi della malattia e come questa può contribuire ad alleviare il carico assistenziale dei caregiver.
