The rehabilitation of the aphasic person aims to promote from the acute stage the fundamental cognitive-communicative and relational skills to achieve the maximum possible degree of recovery and social reintegration. This is why it is crucial that the different professionals in the department share the objectives in their interventions so as not to hinder recovery. Working hypothesis: In the light of these considerations the present draft thesis is born, a survey to analyse whether the communicative-linguistic and relational skills of the departmental staff, for acute and rehabilitation, they are sufficient to ensure effective communication in support of the recovery of aphasia patients' skills, or if it is necessary to propose a specific training course that considers the different professionalities, but encourages a common and shared approach. Materials and methods: After carrying out a research in literature on previous studies with premises, objectives and methods similar to the thesis project, a questionnaire was prepared to evaluate the effectiveness of communication between staff of department and aphasic person, investigating the following areas: theoretical knowledge, communicative-relational skills, aptitudes, possible difficulties in communicative exchange and possible training needs. Completion of the questionnaire is self-reported and mandatory for all applications. The questionnaire consists of 40 binary, multiple-choice and 5-point Likert closed-answer questions about the degree of agreement or time frequency of a given factor. The questionnaire was given through Google Modules to nurses, physiotherapists, occupational therapists, OSS, neurologists, physiatrists, for a total of 56 units, in service at the U.O.C. Recovery and Functional Rehabilitation of the hospital Immacolata Concezione of Piove di Sacco (PD), over time from July 2023 to October 2023 inclusive. The data, collected anonymously, were analysed according to indicators of descriptive statistics, such as percentage and fashion. Results and discussion: 69.64% of staff in the department responded to the questionnaire, obtaining 39 responses from 16 physiotherapists, 10 nurses, 9 OSS, 3 physiatrists and 1 occupational therapist, respectively. No responses from neurologists were recorded. The results show some theoretical gaps, especially for nurses (35.71%) and OSS (32.14%). With regard to practical skills, more than half of the sample considers that they possess valid and effective skills (76.90%), paying attention to communication prerequisites, exploiting facilitative strategies (74.40%) and disposing with a positive attitude towards the person with aphasia (82.10%). However, more than half of the staff, more than 56.40%, find the interaction with this user difficult. In addition, barriers due to policy and organization of the hospital structure are highlighted. Within the reality analyzed the most struggling professionals seem to be the nurses. Limits: The survey was conducted within a single department, so in the future it appears necessary to involve more structures to identify potential and barriers to effective communication related to individual professions. It is also useful to integrate the survey through field observation to verify the veracity of self-assessment. Conclusions: The investigation highlights how the department staff of the U.O.C. RRF of the hospital of Piove di Sacco (PD) undertakes to promote effective communication exchanges with the person with aphasia. Participation in a training course would help implement and standardise staff knowledge and skills, increasing the chances of successful communication exchange with the aphasic individual, and also benefiting the aphasic individual.
La presa in carico riabilitativa della persona afasica mira a promuovere sin dalla fase acuta le abilità cognitivo-comunicative e relazionali fondamentali per raggiungere il massimo grado possibile di recupero e di reinserimento sociale. Per tale motivo risulta di fondamentale importanza la condivisione degli obiettivi negli interventi alla persona da parte dei diversi professionisti del reparto, in modo da attivare e non ostacolare il recupero. Alla luce di tali considerazioni nasce il presente progetto di tesi, un'indagine volta ad analizzare se le competenze comunicativo-linguistiche e relazionali del personale di reparto, per acuti e di riabilitazione, siano sufficienti a garantire una comunicazione efficace a sostegno del recupero delle abilità della persona con afasia, oppure se sia necessario proporre un percorso di formazione specifica che consideri le diverse professionalità, ma favorisca un approccio comune e condiviso. Dopo aver eseguito una ricerca in letteratura su precedenti studi con premesse, obiettivi e metodi similari al progetto di tesi, è stato predisposto un questionario che valuti l’efficacia della comunicazione tra personale di reparto e persona afasica, indagando i seguenti ambiti: conoscenze teoriche, competenze comunicativo-relazionali, attitudini, possibili difficoltà nello scambio comunicativo ed eventuali necessità di formazione. La compilazione del questionario è autoriportata ed obbligatoria per tutte le domande. Il questionario è costituito da 40 domande a risposta chiusa binaria, a scelta multipla e su scala Likert a 5 punti relativa al grado di accordo oppure alla frequenza temporale di un dato fattore. Il questionario è stato somministrato tramite Google Moduli a infermieri, fisioterapisti, terapisti occupazionali, OSS, neurologi, fisiatri, per un totale di 56 unità, in servizio presso l’U.O.C. Recupero e Riabilitazione Funzionale dell’Ospedale Immacolata Concezione di Piove di Sacco (PD), nell’arco di tempo da luglio 2023 a ottobre 2023 incluso. I dati, raccolti in maniera anonima, sono stati analizzati secondo indicatori di statistica descrittiva, quali percentuale e moda. Al questionario ha risposto il 69,64% del personale del reparto, ottenendo 39 risposte rispettivamente da parte di 16 fisioterapisti, 10 infermieri, 9 OSS, 3 fisiatri ed 1 terapista occupazionale. Non sono state registrate risposte da parte dei neurologi. I risultati evidenziano alcune lacune teoriche, soprattutto per infermieri (35,71%) ed OSS (32,14%). Relativamente alle abilità pratiche, più della metà del campione ritiene di possedere competenze valide ed efficaci (76,90%), ponendo attenzione ai prerequisiti alla comunicazione, sfruttando strategie facilitanti (74,40%) e disponendosi con un’attitudine positiva nei confronti della persona con afasia (82,10%). Tuttavia oltre metà del personale, più del 56,40%, reputa faticosa l’interazione con tale utenza. In aggiunta, vengono sottolineate barriere dovute a policy ed organizzazione della struttura ospedaliera. All’interno della realtà analizzata i professionisti più in difficoltà sembrano essere gli infermieri. L’indagine è stata condotta all’interno di un solo reparto, perciò in futuro risulta necessario coinvolgere più strutture per individuare potenzialità e barriere alla comunicazione efficace relative alle singole professioni. Inoltre sorge utile integrare l’indagine tramite un’osservazione sul campo per verificare la veridicità dell’autovalutazione. L’indagine evidenzia come il personale di reparto dell’U.O.C. RRF dell’Ospedale di Piove di Sacco (PD) si impegni a favorire scambi comunicativi efficaci con la persona con afasia. La partecipazione ad un corso di formazione contribuirebbe ad implementare e uniformare conoscenze e competenze del personale, aumentando le possibilità di riuscita dello scambio comunicativo con l’individuo afasico, recando beneficio pure a quest’ultimo.
