Background: bisphosphonates-related osteonecrosis of the jaws (BRONJ) is an adverse-drug reaction manifested by bone destruction and necrosis with or without bone exposure in patients not undergoing radiation therapy. BRONJ’s diagnosis and staging are based on anamnesis and clinical-radiological criteria, specifically identifying 3 stages of disease: focal, diffuse, and complicated. The surgical approach, unlike the medical one, involves the removal of necrotic bone with likely healthy resection margins. Purpose: the study aims to investigate the effectiveness of marginal resection performed with a piezoelectric device in patients who present signs and symptoms of BRONJ and for whom there is no therapeutic alternative to surgical-invasive treatment. Materials and methods: a retrospective study was conducted on subjects who arrived, at the Dental Unit - Azienda Ospedaliera in Padua, from January 2017 to April 2024. In the study cohort were included the patients that have received BRONJ’s histological diagnosis and that have undergone marginal resection of the jaws with piezoelectric device. Patients subjected to head and neck radiotherapy, MRONJ patients, and with primary cancer in the maxillary bones were excluded. All surgeries were performed under local anesthesia, conscious sedation, and by the same first operator-surgeon. The marginal resection with piezoelectric device was considered an effective treatment to achieve complete epithelialization of the surgical site in the absence of symptoms and clinical-radiographic signs of BRONJ. Results: selected 21 patients (17 women and 4 men) for a total of 21surgeries. Performed 14 marginal resections in the lower jaw (5 right-side and 9 left side) and 7 resections in the upper jaw (2 right-side and 5 left-side). In 95% of patients (n = 20), 1 year after surgery, no signs and symptoms of the disease were found. A single clinical case presented recurrence: it occurred first 1 year (2018) and then 2 years (2019) after the first marginal resection dated 2017, both recurrences were treated. Conclusions: the study shows that the marginal resection with piezoelectric device is an effective procedure in the BRONJ’s treatment, however, it remains an invasive and destructive therapeutic approach. Further scientific researches involving more subjects are needed and preferred to confirm the results obtained.
Introduzione: l’osteonecrosi dei mascellari indotta da bisfosfonati (BRONJ) è una reazione farmaco-avversa che si manifesta con distruzione e necrosi ossea con o senza esposizione di osso in pazienti non sottoposti a radioterapia. Diagnosi e stadiazione di BRONJ si basano su anamnesi e su criteri clinico-radiografici, nello specifico si identificano 3 stadi di malattia: focale, diffusa e complicata. L’approccio chirurgico, a differenza di quello medico, comporta l’asportazione dell’osso necrotico con margini di resezione verosimilmente sani. Scopo: Lo studio si propone come scopo di indagare l’efficacia della resezione marginale eseguita tramite dispositivo piezoelettrico in pazienti che presentano segni e sintomi di BRONJ e per i quali non esiste un’alternativa terapeutica rispetto al trattamento chirurgico-invasivo. Materiali e metodi: è stato condotto uno studio retrospettivo su soggetti afferiti, presso la Clinica Odontoiatrica – Azienda Ospedaliera di Padova, da gennaio 2017 ad aprile 2024. Sono stati inclusi, nella corte di studio, i pazienti che hanno ricevuto diagnosi istologica di BRONJ e che sono stati sottoposti a resezione marginale dei mascellari con strumento piezoelettrico. Pazienti sottoposti a radioterapia di testa e collo, pazienti MRONJ e con tumore primitivo delle ossa mascellari sono stati esclusi. Tutte le chirurgie sono state eseguite in anestesia locale, sedazione cosciente e dallo stesso primo operatore-chirurgo. La resezione marginale con piezoelettrico è stata considerata trattamento efficace al raggiungimento della completa epitelizzazione del sito chirurgico in assenza di sintomi e segni clinico-radiografici di BRONJ. Risultati: sono stati selezionati 21 pazienti (17 donne e 4 uomini) per un totale di 21interventi. Sono state eseguite 14 resezioni marginali nel mascellare inferiore (5 a destra e 9 a sinistra) e 7 resezioni nel mascellare superiore (2 a destra e 5 a sinistra). Nel 95% dei pazienti ( n = 20), ad 1 anno dalla chirurgia, non sono stati riscontrati segni e sintomi di malattia. Un singolo caso clinico ha presentato recidiva, verificatasi ad 1 anno (2018) e a 2 anni (2019) di distanza dalla prima resezione marginale datata 2017, entrambe le recidive sono state trattate. Conclusioni: lo studio evidenzia come la resezione marginale con piezoelettrico sia una procedura efficace nel trattamento dell’osteonecrosi farmaco-relata a bisfosfonati, pur rimanendo questa un approccio terapeutico invasivo e demolitivo. Sono necessarie e auspicabili ulteriori ricerche scientifiche che comprendano un numero maggiore di soggetti al fine di confermare i risultati ottenuti.
