BACKGROUND: Telenursing is the latest technology for performing remote blood glucose monitoring. This thesis aims to investigate the methodology and effectiveness of this type of nursing intervention to improve the management of diabetes mellitus at home. The latter is motivated by the fact of reducing hospitalizations of patients with this chronic disease and allowing most people to access health services easily directly from home. OBJECTIVES: This study aims to critically examine the role of telenursing interventions in the management of diabetes mellitus; highlighting the outcomes on glycemic control, glycated hemoglobin, adherence to therapy, dietary regimen; and finally, evaluating the degree of satisfaction of patients to whom this type of intervention was addressed. METHODS: The Pubmed database was consulted for the research. The research is based on the analysis of data and research in the field of telenursing applied to the home management of diabetes mellitus. RESULTS: Studies have shown that in the groups in which telenursing nursing interventions were carried out, positive results were measured regarding the decrease in glycated hemoglobin and glycemia, greater adherence to therapy and diet, and a fair degree of satisfaction, by patients, with the interventions they underwent. CONCLUSION: Telenursing represents a valid tool to increase self-efficacy in the home management of diabetes mellitus and reduce glycemia; however, specific training of a multidisciplinary team is necessary to deal with the organization and management of these patients. Future research should focus on the validation of a specific and common intervention methodology, on the application of interventions with a longer duration, in larger cohorts and should focus on other areas of diabetes management; such as, for example, quality of life, quality of care and cost effectiveness. BACKGROUND: Il telenursing rappresenta la tecnologia più recente per effettuare il monitoraggio della glicemia da remoto. Questa tesi si propone si indagare la metodologia e l’efficacia di questo tipo di intervento infermieristico per migliorare la gestione del diabete mellito a domicilio. Quest’ultima è motivata dal fatto di ridurre le ospedalizzazioni di pazienti con questa patologia cronica e permettere alla maggior parte delle persone di accedere ai servizi sanitari in modo semplice direttamente dal domicilio. OBIETTIVI: Questo studio ha lo scopo di esaminare in maniera critica il ruolo degli interventi di telenursing nella gestione del diabete mellito; dando rilievo agli esiti sul controllo glicemico, sull’emoglobina glicata, sull’aderenza alla terapia, al regime dietetico; e infine, valutando il grado di soddisfazione dei pazienti a cui sono stati rivolti questa tipologia di interventi. METODI: Per la ricerca è stato consultato il database Pubmed. La ricerca si basa sull’analisi di dati e ricerche nel campo del telenursing applicato alla gestione domiciliare del diabete mellito. RISULTATI: Dagli studi è emerso che nei gruppi in cui si svolgevano interventi infermieristici di telenursing, si misuravano risultati positivi per quanto riguarda la diminuzione dell’emoglobina glicata e della glicemia, una maggiore aderenza alla terapia e alla dieta, e un discreto grado di soddisfazione, da parte dei pazienti, agli interventi a cui si sono sottoposti. CONCLUSIONE: Il telenursing rappresenta un valido strumento per aumentare l’autoefficacia della gestione domiciliare del diabete mellito e ridurre la glicemia; è necessaria però che vi sia una formazione specifica di un team multidisciplinare che si occupi dell’organizzazione e della gestione di questi pazienti. Le ricerche future dovrebbero concentrarsi sulla validazione di una metodologia di intervento specifica e comune, sull’applicazione di interventi con una durata maggiore, in coorti più grandi e dovrebbero concentrarsi in altre aree della gestione del diabete; come, per esempio, la qualità di vita, la qualità delle cure e l’efficacia dei costi.
