ABSTRACT In recent years, the debate on overcoming restraint in Psychiatric Diagnostic and Treatment Services (P.D.S.) has become increasingly central, but only a few departments have decided to abandon its use, although the results obtained by those who have adopted this practice are significant. This thesis aims to highlight the critical issues associated with the practice of restraint and, above all, to explore possible alternatives. Particular attention is paid to the inclusion and the role that the Psychiatric Rehabilitation Technician (P.R.T.) can play in reducing in-patient aggression by implementing strategies aimed at preventing aggressive escalation and replacing the use of restraint. Finally, an experimental project implemented at the Mestre SPDC is presented, aimed at managing patient stress and promoting anger management. OBJECTIVES The objective of this thesis is to understand the meaning and implications of restraint, analysing the critical issues and consequences of this practice. It aims to explore a set of possible alternatives to the use of restraint, focusing on the inclusion of the figure of the Psychiatric Rehabilitation Technician as a key element in the management of patient aggression. It also aims to provide an overview of the practical solutions available, highlighting strategies that can replace restraint in care settings. MATERIALS AND METHODS Initially, a literature review was conducted to build a solid theoretical basis on the topic of restraint. Subsequently, a description of different SPDCs was provided through the analysis of experiences and first-hand accounts. The thesis was supplemented by consulting scientific articles and relevant academic material. Finally, an experimental project developed during the internship was presented, aimed at proposing intervention strategies for stress management. RESULTS AND CONCLUSIONS The conclusions show how the inclusion of a professional figure dedicated to rehabilitative interventions, such as the Psychiatric Rehabilitation Technician, could contribute to a significant reduction in episodes of aggression and, consequently, decrease the need to resort to restraint practices. The strategies learnt by the patients during the meetings prove to be effective in improving the management of difficulties and preventing crises.
ABSTRACT Negli ultimi anni, il dibattito sul superamento della contenzione nei Servizi Psichiatrici di Diagnosi e Cura (S.P.D.C.) è diventato sempre più centrale, ma solo pochi reparti hanno deciso di abbandonarne l'uso, sebbene i risultati ottenuti da chi ha adottato questa pratica siano significativi. Questa tesi si propone di evidenziare le criticità associate alla pratica della contenzione e, soprattutto, di esplorare le alternative possibili. Particolare attenzione è rivolta all'inserimento e al ruolo che il Tecnico della Riabilitazione Psichiatrica (Te.R.P.) può svolgere nella riduzione dell'aggressività del paziente ricoverato, implementando strategie mirate a prevenire l'escalation aggressiva e a sostituire l’uso della contenzione. Infine, viene presentato un progetto sperimentale realizzato presso l'SPDC di Mestre, volto a gestire lo stress dei pazienti e a promuovere la gestione della rabbia OBIETTIVI L'obiettivo di questa tesi è comprendere il significato e le implicazioni della contenzione, analizzando le criticità e le conseguenze di tale pratica. Si intende esplorare un insieme di possibili alternative all'uso della contenzione, focalizzandosi sull'inserimento della figura del Tecnico della Riabilitazione Psichiatrica come elemento chiave nella gestione dell’aggressività del paziente. Inoltre, si mira a fornire una panoramica delle soluzioni pratiche a disposizione, evidenziando le strategie che possono sostituire la contenzione nei contesti di cura. MATERIALI E METODI Inizialmente, è stata condotta una revisione della letteratura per costruire una solida base teorica sul tema della contenzione. Successivamente, è stata fornita una descrizione di diversi SPDC attraverso l'analisi di esperienze e testimonianze dirette. La tesi è stata integrata con la consultazione di articoli scientifici e materiale accademico pertinente. Infine, è stato presentato un progetto sperimentale sviluppato durante il tirocinio, finalizzato a proporre strategie di intervento per la gestione dello stress. RISULTATI E CONCLUSIONI Le conclusioni mostrano come l'inserimento di una figura professionale dedicata agli interventi riabilitativi, come il Tecnico della Riabilitazione Psichiatrica, potrebbe contribuire a una riduzione significativa degli episodi di aggressività e, di conseguenza, diminuirebbe la necessità di ricorrere a pratiche di contenzione. Le strategie apprese dai pazienti durante gli incontri si rivelano efficaci nel migliorare la gestione delle difficoltà e nel prevenire le crisi.
