Background: Chronic wounds represent one of the main challenges in the clinical management of skin lesions, particularly due to their difficult healing and the high risk of infectious complications. The treatment of these wounds requires an approach that considers the specific conditions of the patient and the evolution of the wound. Medical honey has garnered interest for its antibacterial properties and ability to promote healing. Materials and Methods: A 78-year-old male patient with a suppurating post-traumatic lesion on the right lower limb, unresponsive to systemic antibiotic therapy. Treatment involved surgical debridement and cleansing, with an advanced dressing protocol that included medical honey. The TIMERS method was used to assess and monitor the wound, along with the clock method to describe the location and progression of undermining. Data collection was conducted through access to clinical documentation, authorized by the relevant corporate offices of ULSS1 Dolomiti. Results: There was progressive improvement following the introduction of medical honey, with significant reduction in undermining and infection. After 10 days, the lesion transitioned from the inflammatory phase to the proliferative phase. After 105 days of treatment, the wound was completely healed, with stable scarring and no recurrences. Conclusions: Medical honey proved effective in the treatment of the infected and cavitary wound in combination with other advanced dressings. The case suggests that cleansing, antisepsis, and topical treatment with advanced dressings, particularly in colonizations and inflammations that do not respond to systemic antibiotics, may offer an alternative in clinical practice. Background: le lesioni croniche rappresentano una delle principali sfide nella gestione clinica delle lesioni cutanee, soprattutto per la loro difficile guarigione e l'elevato rischio di complicanze infettive. Il trattamento di queste lesioni richiede un approccio che consideri le condizioni specifiche del paziente e l'evoluzione della ferita. Il miele medicale è stato oggetto di interesse per le sue proprietà antibatteriche e promuoventi la guarigione, Materiali e metodi: paziente uomo di 78 anni, portatore di lesione posttraumatica suppurata all'arto inferiore destro, non responsiva alla terapia antibiotica sistemica. Trattamento tramite debridement chirurgico e detersione, protocollo di medicazione avanzata, includendo miele medicale. Il metodo TIMERS è stato utilizzato per valutare e monitorare la lesione, insieme al metodo dell’orologio per descrivere la posizione e l'evoluzione delle sottominature. Raccolta dati svolta attraverso accesso alla documentazione clinica, previa autorizzazione da parte degli uffici aziendali competenti ULSS1 Dolomiti. Risultati: miglioramento progressivo dopo l'introduzione del miele medicale, con riduzione significativa delle sottominature e dell'infezione. Dopo 10 giorni la lesione è uscita dalla fase infiammatoria ed è entrata in quella proliferativa. Dopo 105 giorni di trattamento la ferita era completamente guarita, con cicatrizzazione stabile e senza recidive. Conclusioni: Il miele medicale si è dimostrato efficace nel trattamento della lesione infetta e cavitaria in combinazione con altre medicazioni avanzate. Il caso suggerisce che la detersione, l'antisepsi e il trattamento topico con medicazioni avanzate in particolare nelle colonizzazioni e nelle infiammazioni che non rispondono agli antibiotici sistemici può offrire un'alternativa nella pratica clinica.
