Introduction: Beyond nicotine addiction as a biological phenomenon, it is necessary to consider the psychological and social aspects that contribute to the meaning associated with smoking behavior. Those who suffer from mental disorders are particularly vulnerable to smoking addiction, with higher rates of morbidity and mortality compared to the general population. For this reason, understanding the underlying factors of smoking behavior is essential for structuring more effective cessation interventions. However, there is still little knowledge about how smokers with psychiatric disorders experience such interventions, the therapeutic alliance, and, consequently, what meaning they attribute to alternatives to conventional cigarettes. Methods: The study participants are smokers with mental disorders who are taking part in the harm reduction intervention "Genesis Trial," a 6-month program that promotes the use of electronic cigarettes, and they reflect on their experience through semi-structured interviews. The qualitative data will then be analyzed using Reflexive Thematic Analysis. Results: In the absence of preliminary results, researchers hypothesize that participants may report the program's influence on their smoking behavior and, in particular, on the role of the therapeutic alliance. In this context, the alternative to conventional cigarettes could be perceived as a transitional object, reminding participants of their shared commitment to improving their health. Discussion: This study contributes to understanding participants' experiences with the program, the importance of the therapeutic alliance with mental health professionals, and its influence on the attribution of meaning to alternatives to conventional cigarettes, such as electronic cigarettes.
Introduzione: Oltre alla dipendenza da nicotina come fenomeno biologico, è necessario considerare gli aspetti psicologici e sociali che contribuiscono alla costruzione del significato associato al comportamento di fumo. Coloro che soffrono di disturbi mentali sono particolarmente vulnerabili alla dipendenza dal fumo, con tassi più elevati di morbilità e mortalità rispetto alla popolazione generale. Per questo motivo capire cosa sottostà al comportamento del fumo è essenziale per strutturare degli interventi di cessazione maggiormente efficaci. Tuttavia, c'è ancora poca conoscenza su come i fumatori che soffrono di disturbi psichiatrici vivano tali interventi, l'alleanza terapeutica e, di conseguenza, quale significato attribuiscano alle alternative alla sigaretta convenzionale. Metodologia: I partecipanti dello studio sono fumatori con disturbi mentali che prendono parte all'intervento di harm reduction "Genesis Trial", un programma di 6 mesi che promuove l'uso della sigaretta elettronica, e riflettono sulla loro esperienza tramite interviste semi-strutturate. I dati qualitativi saranno poi analizzati con l'Analisi Tematica Riflessiva. Risultati: In assenza di risultati preliminari, i ricercatori ipotizzano che i partecipanti potrebbero riportare l'influenza del programma sul loro comportamento di fumo e, in particolare, sul ruolo dell'alleanza terapeutica. In tale contesto, l'alternativa alla sigaretta convenzionale potrebbe essere vissuta come un oggetto transizionale, richiamando ai partecipanti l'impegno comune nel migliorare la loro salute. Discussione: Questo studio contribuisce alla comprensione delle esperienze dei partecipanti del programma, dell'importanza dell'alleanza terapeutica con i professionisti della salute mentale e della sua influenza sull'attribuzione di significato alle alternative alle sigarette convenzionali, come le sigarette elettroniche.
