Background. Nursing in the context of mental health is a complex challenge, as it involves not only clinical and therapeutic aspects but also significant ethical dilemmas. Psychiatric patients, often in states of extreme vulnerability, require care that respects their fundamental rights, such as the right to dignity, autonomy, and informed consent. However, nurses frequently face situations where these ethical principles must be balanced with the need to ensure the safety of the patient and others, especially in emergency situations or when urgent action is required. This study explores the role of ethics in nursing care for patients with mental disorders, aiming to identify the main challenges and perspectives through a narrative literature review. Objective. The primary objective of this thesis is to analyze how ethics influences nursing decisions and practices in the care of psychiatric patients. Through a literature review, the ethical challenges that arise in psychiatric care will be examined, with particular attention to respecting patient dignity, managing moral dilemmas, and implementing person-centered care. Strategies for improving the therapeutic relationship and promoting a more ethical nursing practice will also be discussed. Materials and Methods. The research is based on a narrative literature review, conducted through the analysis of scientific articles retrieved from the MEDLINE database via PubMed. Results. The results of the review indicate that nurses working in the psychiatric field encounter daily ethical dilemmas related to the care of vulnerable patients. Elements such as empathy, acknowledgment of dignity, and active listening emerge as fundamental to building an effective therapeutic relationship; however, their application is often hindered by limited resources and situations where it is necessary to balance respect for the patient's autonomy with the need to ensure their safety and that of others. Furthermore, the review highlights the importance of a multidisciplinary approach, which allows ethical challenges to be addressed in an integrated manner, ensuring holistic and personalized care. Conclusions. Nursing care for psychiatric patients requires deep ethical reflection. Nurse training, along with greater awareness of ethical principles, is essential to ensure care that fully respects the dignity and autonomy of the patient, even in the most challenging situations. Ethical challenges must be addressed through dialogue and collaboration among all involved professionals, promoting an approach based on active listening and empathy. In the future, increased attention to the ethical dimension of nursing care could help improve the quality of care provided to psychiatric patients while ensuring respect for their rights. Keywords. Background. L’assistenza infermieristica nel contesto della salute mentale rappresenta una sfida complessa, poiché coinvolge non solo aspetti clinici e terapeutici, ma anche importanti dilemmi etici. I pazienti psichiatrici, spesso in condizioni di estrema vulnerabilità, necessitano di cure che rispettino i loro diritti fondamentali, come il diritto alla dignità, all'autonomia e al consenso informato. Tuttavia, gli infermieri si trovano frequentemente ad affrontare situazioni in cui tali principi etici devono essere bilanciati con la necessità di garantire la sicurezza del paziente e degli altri, in particolare in situazioni di emergenza o stato di necessità. Questo studio esplora il ruolo dell'etica nell'assistenza infermieristica a pazienti con disturbi mentali, cercando di identificare le principali sfide e prospettive attraverso una revisione narrativa della letteratura. Obiettivo. L'obiettivo principale di questa tesi è analizzare come l'etica influenzi le decisioni e le pratiche infermieristiche nell'assistenza a pazienti psichiatrici. Attraverso una revisione della letteratura, verranno esaminate le sfide etiche che emergono nell'ambito dell'assistenza psichiatrica, con particolare attenzione al rispetto della dignità del paziente, alla gestione dei dilemmi morali e all'implementazione di un'assistenza centrata sulla persona. Saranno inoltre discusse strategie per migliorare la relazione terapeutica e promuovere una pratica infermieristica più etica. Materiali e metodi. La ricerca si basa su una revisione narrativa della letteratura, condotta attraverso l'analisi di articoli scientifici reperiti dalla banca dati MEDLINE via Pubmed. Risultati. I risultati della revisione indicano che l'infermiere che lavora in ambito psichiatrico si trova quotidianamente a gestire dilemmi etici legati alla cura di pazienti vulnerabili. Elementi come l'empatia, il riconoscimento della dignità e l'ascolto attivo emergono come fondamentali per costruire una relazione terapeutica efficace, nonostante ciò, la loro applicazione è spesso ostacolata da risorse limitate e da situazioni in cui è necessario bilanciare il rispetto dell'autonomia del paziente con l'esigenza di garantire la sua sicurezza e quella degli altri. Inoltre, la revisione evidenzia l'importanza di un approccio multidisciplinare, che permetta di affrontare le sfide etiche in maniera integrata, garantendo un'assistenza olistica e personalizzata. Conclusioni. L'assistenza infermieristica ai pazienti psichiatrici richiede una profonda riflessione etica. La formazione degli infermieri, insieme a una maggiore consapevolezza dei principi etici, è essenziale per garantire un'assistenza che rispetti pienamente la dignità e l'autonomia del paziente, anche nelle situazioni più difficili. Le sfide etiche devono essere affrontate attraverso il dialogo e la collaborazione tra tutte le figure professionali coinvolte, promuovendo un approccio basato sull'ascolto attivo e sull'empatia. In futuro, una maggiore attenzione alla dimensione etica dell'assistenza infermieristica potrebbe contribuire a migliorare la qualità della cura fornita ai pazienti psichiatrici, assicurando al contempo il rispetto dei loro diritti.
