This systematic literature review aims to map and synthesise studies on therapists' emotional responses to patients diagnosed with antisocial personality disorder (ASPD) or psychopathy, taking into consideration the severity of their symptoms and clinical criteria for diagnosis. The therapeutic relationship and countertransference dynamics experienced by individuals with ASPD or psychopathy can have a significant impact on the outcome of treatment. Despite their importance, therapist emotional reactions and response patterns to patients with ASPD or psychopathy are understudied, emphasising the need for further understanding in this important field. The review features observational or controlled studies that use standardised tools to study the emotional response patterns of health professionals (e.g., psychiatrists, clinical psychologists, and social workers) in their clinical practices with individuals diagnosed with ASPD or psychopathy. The search includes papers published in English and available on PubMed, PsycINFO, Web of Science, and ProQuest spanning from 1990 to December 6, 2023. The review integrates findings gathered from various methodologies, samples, and cultural contexts, in a narrative summary. The objective of this systematic literature review is to provide a comprehensive understanding of therapists' emotional responses to patients with ASPD or psychopathy, which can be used to improve clinical practice, training, and future research in this field.
